小探头超声内镜联合多层螺旋CT对胃癌术前分期的研究

摘要目的研究小探头超声内镜联合多层螺旋CT对胃癌术前分期的效果，为临床提供指导。方法选取2016年1月～2017年1月来我院就医的65例经临床病理确诊的胃癌患者作为研究对象，其中25例患者实施小探头超声内镜检查，20例患者实施多层螺旋CT检查，20例胃癌患者实施小探头超声内镜联合多层螺旋CT检查，以术后病理结果为金标准，研究对比小探头超声内镜联合多层螺旋CT诊断胃癌术前分期的效果。结果小探头超声内镜对胃癌T1～4期的准确率分别为100.0%、71.4%、83.3%、66.7%，对胃癌N0～2期的准确率分别为100.0%、75.0%、40.0%；多层螺旋CT检查对胃癌T1～4期的准确率分别为50.0%、50.0%、90.0%、75.0%，对胃癌N0～2期的准确率分别为66.7%、60.0%、71.4%；小探头超声内镜联合多层螺旋CT检查对胃癌T1～4期的准确率分别为100.0%、80.0%、87.5%、100.0%，对胃癌N0～2期的准确率分别为100.0%、90.0%、83.3%。结论小探头超声内镜、多层螺旋CT诊断胃癌术前分期各有其优势，为保证确诊率，应对患者实施联合诊断。

Abstract：ObjectiveTo study the effect ofmini-probe endoscopic ultrasonography combined with multi-slice spiral CT on preoperative staging of gastric cancer,and to provide guidance for clinical practice.M ethods65 patients with gastric cancer diagnosed by clinical pathology who were admitted to our hospital from January 2016 to January 2017 were selected as the study subjects,25 patients with gastric cancer were given mini-probe endoscopic ultrasonography,and 20 patientswith gastric cancer were given multi-slice spiral CT,20 patients with gastric cancer were given miniprobe endoscopic ultrasonography combined with multi-slice spiral CT.The postoperative pathological resultswere taken as the gold standard.The effect ofmini-probe endoscopic ultrasonography combined with multi-slice spiral CT in the diagnosis of gastric cancer was investigated and compared.ResultsThe accuracy ofmini-probe endoscopic ultrasonography on gastric cancer of stage T1-4was 100.0%,71.4%,83.3%and 66.7%respectively.The accuracy on gastric cancer of stage N0-2was 100.0%,75.0%and 40.0%respectively.The accuracy ofmulti-slice spiral CT on gastric cancer of stage T1-4was 50.0%,50.0%,90.0%,75.0%respectively.The accuracy on gastric cancer of stage N0-2was 66.7%,60.0%and 71.4%respectively.The accuracy ofmini-probe endoscopic ultrasonography combined with multi-slice spiral CT on gastric cancer of stage T1-4was 100.0%,80.0%,87.5%and 100.0%respectively.The accuracy of gastric cancer of stage N0-2was 100.0%,90.0%and 83.3%respectively.ConclusionMini-probe endoscopic ultrasonography and multislice spiral CT in the diagnosis of preoperative staging of gastric cancer have different advantages.In order to ensure the diagnosis rate,the patients should be given combined diagnosis.
