腹腔镜与开腹手术治疗阑尾周围脓肿的效果对比

摘要目的探讨腹腔镜与开腹手术治疗阑尾周围脓肿的效果。方法回顾性分析我院普外科2014年2月～2017年2月收治的96例阑尾周围脓肿患者的临床资料，依据手术治疗方式不同分为开腹组（46例）和腹腔镜组（50例）。观察两组的手术时间、术中出血量、24 h镇痛药使用率、住院时间、排气时间、体温恢复时间、白细胞恢复正常时间和切口感染、腹腔内脓肿、盆腔内脓肿发生率及复发率。结果腹腔镜组的术中出血量少于开腹组，24 h镇痛药使用率低于开腹组，住院时间短于开腹组，差异有统计学意义（P＜0.05）。腹腔镜组的排气时间、体温恢复时间、白细胞恢复正常时间均短于开腹组，差异有统计学意义（P＜0.05）。腹腔镜组的切口感染、腹腔内脓肿、盆腔内脓肿发生率及复发率均低于开腹组，差异有统计学意义（P＜0.05）。结论腹腔镜治疗阑尾周围脓肿患者创伤小，恢复快，并发症少，值得临床推广应用。

Abstract：ObjectiveTo explore the effect of laparoscope and open surgery in the treatment of peri-appendiceal abscess.Methods96 cases clinical data of peri-appendiceal abscess in our hospital of general surgery department from February 2014 to February 2017 were retrospective analyzed,all patients were divided into the laparotomy group(46 cases)and the laparoscope group (50 cases)according to different operation treatment methods.The operation time,intraoperatve blood soss,postoperative exhaust time,24 h analgesic usage,hospital stays,exhaust time,body temperature recovery time,white blood cells back to normal time,and the incidence rate of infection of incisional wound,intra-abdominal abscess,pelvic within abscess,recurrence rate of the two groups were observed.ResultsThe bleeding volume in the laparoscopic group was less than that in the laparotomy group,and the use rate of 24 h analgesic in laparoscopic group was lower than that in the laparotomy group,hospital stay in the laparoscopic group was shorter than that in the laparotomy group,with significant difference (P＜0.05).The exhaust time,the recovery time of body temperature and the recovery time of white blood cells in the laparoscopic group was shorter than that in the laparotomy group,with significant difference(P＜0.05).The incidence rate of incision infection,intra-abdominal abscess,pelvic abscess and recurrence rate in the laparoscopic group was lower than that in the laparotomy group,with significant difference (P＜0.05).ConclusionLaparoscope in the treatment of patients with appendiceal abscess has the advantages of less trauma,faster recovery, fewer complications,it is worthy of clinical promotion and application.
