Abstract：Objective To study the clinical effect of holmium laser combining with choledochoscope in the treatment of complicated intra- and extrahepatic bile duct stone and explore its clinical applicability. Methods From June 2013 to June 2015，106 patients who were suffered from complicated intra- and extrahepatic bile duct stone visited out hospital for treatment were selected as research objects.They were divided into control group and experimental group in random. In the control group，conventional laparotomy was applied，while in the experimental group，stones were crushed by holmium laser under choledochoscope，and these crushed stones were removed by basket extractor or rushed out by water flooding.The operation time，intraoperative blood loss，hospital stay，postoperative ventilation time，calculi residual rate，success rate of surgery and postoperative complications in two groups of patients after treatment were compared. Results The operation time in the experimental group were much longer in comparison with that in the control group.The intraoperative amount of bleeding，hospital stay，and postoperative ventilation time were much less than those in the control group with statistical differences（P＜0.05）.In the experimental group，the residual stone rate was 5.66%，much lowerer than that in the control group accounting for 18.87%.The surgical success rate in the experimental group was 98.11%，greatly higher than that in the control group accounting for 73.58%.The incidence of complications after surgery in the experimental group was 3.77%，much lower in comparison with that in the control group accounting for 15.09%with statistical differences（P＜0.05）. Conclusion Combination of holmium laser and choledochoscope in the treatment of complicated intra- and extrahepatic bile duct stone is accurate and intuitive with a definite effect and high safety，which is worthy of clinical promotion and application.
