内镜下黏膜剥离术联合内镜下套扎术治疗胃小间质瘤的临床效果

摘要目的探讨内镜下黏膜剥离术联合内镜下套扎术治疗胃小间质瘤的临床效果。方法选取2017 年1 月至2020 年3 月九江学院附属医院收治的胃小间质瘤患者56 例作为研究对象，采用随机纸片法分为对照组（n=28）和观察组（n=28）。对照组采用内镜下黏膜切除术治疗，观察组采用内镜下黏膜剥离术联合内镜下套扎术治疗。比较两组患者临床指标、临床疗效、预后状况。结果观察组术中出血量少于对照组，住院时间短于对照组，手术时间长于对照组，病灶切除面积大于对照组，差异有统计学意义（P<0.05）。观察组整体切除率、治愈性切除率均高于对照组，差异有统计学意义（P<0.05）。两组胃穿孔或胃出血率、复发率、肝脏转移率、死亡率比较，差异无统计学意义（P>0.05）。结论内镜下黏膜剥离术联合内镜下套扎术治疗胃小间质瘤，创伤小，恢复快，预后良好。

Abstract：Objective To explore the clinical effect of endoscopic mucosal dissection combined with endoscopic band ligation in the treatment of small gastric stromal tumors. Methods Fifty-six patients with small gastric stromal tumors admitted to the Affiliated Hospital of Jiujiang University from January 2017 to March 2020 were selected as the research objects, and were divided into a control group (n=28) and an observation group (n=28) by random paper method，the control group was treated with endoscopic mucosal resection, and the observation group was treated with endoscopic mucosal dissection combined with endoscopic band ligation. The clinical indicators, clinical efficacy, and prognosis of the two groups of patients were compared. Results The intraoperative blood loss of the observation group was less than that of the control group, the hospital stay was shorter than that of the control group, the operation time was longer than that of the control group, and the area of lesion resection was larger than that of the control group, the differences were statistically significant (P<0.05). The overall resection rate and curative resection rate in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the rate of gastric perforation or gastric bleeding, recurrence rate, liver metastasis rate,and mortality rate between the two groups (P>0.05). Conclusion Endoscopic mucosal dissection combined with endoscopic band ligation for the treatment of small gastric stromal tumors has small trauma, quick recovery and good prognosis.
