Abstract:Objective To explore the clinical effect of endoscopic dissection/resection in the treatment of duodenal neuroendocrine tumors (D-NENS).Methods A total of 83 patients with D-NENS admitted to General Hospital of Northern Theater Command from January, 2012 to July, 2017 were selected as the research objects.The patients were divided into the dissection group (n=41) and the resection group (n=42) according to the random number table method.Endoscopic mucosal resection (EMR) was performed in the excision group, and endoscopic mucosal dissection (EMD) was performed in the dissection group.The perioperative indexes, postoperative complications, changes of serum inflammatory factors before and after surgery, postoperative recurrence rate and mortality were analyzed in the two groups.Results There was no significant difference in tumor resection rate between the two groups (P>0.05).The duration of operation and postoperative hospital recovery in the dissection group were shorter than those in the excision group, with statistical significance differences (P<0.05).The total incidence of postoperative complications in the dissection group was lower than that in the excision group, and the difference was statistically significant (P<0.05).There were no significant differences in the levels of IL-6 and CRP between the two groups before and 3 days after operation (P>0.05).At postoperative 3 days, the levels of IL-6 and CRP in the two groups were higher than those before surgery, and the differences were statistically significant (P<0.05).There was no significant difference in postoperative recurrence rate and mortality between the two groups (P>0.05).Conclusion The clinical effect and prognosis of EMD and EMR in the treatment of D-NENS are similar, but the operative time and postoperative hospital stay of EMD are shorter, and the risk of postoperative complications is lower, which is worthy of clinical promotion and application.
