基于INS标准的护理干预在儿呼吸科外周静脉留置针维护中的应用

摘要目的探讨基于《输液治疗实践标准》（INS，美国静脉输液护理学会2016年编制）制定的护理干预在儿呼吸科外周静脉留置针维护的应用效果。方法选取2019年5月至11月珠海市妇幼保健院收治的688 例留置外周静脉留置针患儿为研究对象，按照入院时间进行分组。将2019年5月至8月收治的患儿分为对照组（342 例），进行常规外周静脉留置针维护护理；将2019年9月至11月收治的患儿分为观察组（346 例）。在总结并分析患儿外周静脉留置针维护期导管回血、渗血、敷料卷边或松脱、留置针固定不规范及静脉炎的发生情况及发生原因的基础上，结合INS 标准制定相对应的护理措施干预。比较两组的静脉留置针留置时间和不良反应的发生率。结果观察组的静脉留置针留置时间长于对照组，导管回血、敷料卷边或松脱、留置针固定不规范及静脉炎的发生率低于对照组，差异均有统计学意义（P＜0.05）。两组静脉留置针渗血的发生率比较，差异无统计学差异（P＞0.05）。结论基于INS标准制定患儿留置针护理干预措施更好地减少患儿留置针维护期不良反应发生率，提高静脉留置的安全性，有效延长静脉留置针留置时间。

Abstract：Objective To explore the application effect of nursing intervention based on the Infusion Therapy Standards of Practice(infusion nurses society[INS]standards,compiled by the INS in 2016)in the maintenance of peripheral venous indwelling needles in pediatric respiratory department.Methods A total of 688 children received indwelling peripheral intravenous needles who were admitted to Zhuhai Maternal and Child Health Hospital were selected as the research subjects.They were grouped according to the time of admission: the children admitted from May to August 2019 were set as the control group(n=342),and routine maintenance and care of peripheral vein indwelling needles were performed;the children admitted from September to November 2019 were set as the observation group(n=346),corresponding intervention was provided with the reference to the standards formulated by the INS after summarizing and analyzing the occurrence and causes of catheter blood return,errhysis,dressings coverslipped or loose,non-standardized fixation of indwelling needle,and phlebitis during the maintenance of intravenous indwelling needles.The indwelling time and the incidence of adverse reactions between the two groups were compared.Results The indwelling time of the venous indwelling needle in the observation group was longer than that in the control group,the incidence of blood return,dressings coverslipped or loose,irregular fixation of the indwelling needle,and phlebitis in the observation group were lower than those in the control group,the differences were statistically significant(P＜0.05).There was no significant difference in the incidence of intravenous indwelling needle errhysis between the two groups(P＞0.05).Conclusion The nursing intervention for the maintenance of indwelling needles in children based on INS standards can better reduce the incidence of adverse reactions during the maintenance of indwelling needles in children,increase the safety of indwelling veins,and effectively prolong the indwelling time of indwelling needles.
