Abstract:Objective To investigate the causes of weak agglutination on the secondary side of cross matching with micro-column gel test(MGT)and the countermeasures of matching.Methods A total of 3606 blood samples collected from September 2019 to October 2020 in Yunfu People′s Hospital were included in the study.The direct anti-human globulin test(DAT)was performed on the samples with weak agglutination on the secondary side of MGT cross-matching blood,and the antibody screening test was performed on donors.The causes of weak agglutination on the secondary side of MGT cross-matching blood were analyzed,and the corresponding matching countermeasures were discussed.Results There were 120 cases of secondary weak agglutination in the 3606 blood samples,including 117 cases of DAT positive,2 cases of positive donor antibody screening,and 1 case of false agglutination.Further immunophenotyping and diffusion tests were performed on the five DAT-positive specimens with obvious jaundice.The results showed that all five specimens were IgG-type,four were all negative for antibody screening after diffusion,and one was all positive for antibody screening after diffusion.Conclusion 97.50% of the cases of weak agglutination on the secondary side of MGT cross-matching blood are caused by DAT positivity in patients,and the agglutination intensity on the secondary side of cross-matching blood is not stronger than that of DAT in patients.For the specimen with DAT positive and obvious jaundice,in order to improve the safety and effectiveness of blood transfusion,the causes should be found and the corresponding blood matching scheme should be taken.
