摘要目的 探讨阑尾残端不同处理方式对单孔腹腔镜阑尾切除术效果、安全性的影响。方法 选取2019年1月—2020年11月瑞金市人民医院外一科收治的66 例单孔腹腔镜阑尾切除术患者作为研究对象。采用随机摸球法分为观察组（n=33）和对照组（n=33），对照组患者采用生物夹结扎法，观察组患者采用荷包缝合法。比较两组患者的住院时间、术后通气时间、手术时间、术中出血量、术后8 h 疼痛评分及并发症发生情况。结果 观察组患者的住院时间、术后通气时间均短于对照组，手术时间长于对照组，差异有统计学意义（P<0.05）；两组患者的术中出血量、术后8 h 疼痛评分比较，差异无统计学意义（P>0.05）；观察组患者的并发症总发生率（3.03%）低于对照组（18.18%），差异有统计学意义（P<0.05）。结论 阑尾残端采用生物夹结扎法、荷包缝合法处理均具有明显效果，荷包缝合法患者的恢复速度快速，并发症少，但手术时间长，可根据患者情况选取合理的处理方法。
Abstract：Objective To explore the influence on the effect and safety of single port laparoscopic appendectomy with different treatment methods of appendix stump. Methods A total of 66 patients with single port laparoscopic appendectomy admitted to the First Department of Surgery of Ruijin People′s Hospital from January 2019 to November 2020 were selected as the research subjects. They were divided into observation group (n=33) and control group (n=33) by random touch ball method. Patients in the control group were treated with biological clip ligation and patients in the observation group were treated with purse bag suture. The length of stay, postoperative ventilation time, operative time,intraoperative blood loss, postoperative pain score at 8 h and the incidence of complications were compared between the two groups. Results The duration of hospital stay and postoperative ventilation in the observation group were shorter than those in the control group, and the operation time was longer than that in the control group, the differences were statistically significant (P<0.05). There were no significant differences in intraoperative blood loss and postoperative pain score at 8 h between the two groups (P>0.05). The total incidence rate of complications in observation group(3.03%) was lower than that in control group (18.18%), and the difference was statistically significant (P<0.05). Conclusion The treatment of appendix stump with biological clip ligation and purse string suture has obvious effect. Purse string suture has the advantages of rapid recovery, fewer complications and long operation time, so the reasonable treatment method can be chosen according to the situation of patients.
