摘要目的 探讨早期母婴皮肤接触对新生儿母乳喂养情况和血糖的影响。方法 选取2019年4~11月深圳市福田妇幼保健院产科收治的80 例产妇和新生儿为研究对象,按照简单随机化法分为对照组(n=40,予以常规产科干预)和观察组(n=40,予以早期皮肤接触),观察两组新生儿血糖、喂养情况及产妇泌乳和子宫复旧情况。结果 干预后,观察组新生儿血糖水平高于对照组,低血糖发生率低于对照组,差异有统计学意义(P<0.05);观察组产后30 min 内启动母乳喂养率、90 min 内完成母乳喂养率、出院前纯母乳喂养率及3 个月纯母乳喂养率均高于对照组,差异有统计学意义(P<0.05);观察组泌乳启动时间短于照组,观察组产后24、48 h 子宫底均较本组产后2 h 下降,但观察组产后24 h 和产后48 h 子宫下降高度均大于对照组,差异有统计学意义(P<0.05)。结论 早期母婴皮肤接触可降低新生儿低血糖发生率,提高母乳喂养效率及促进泌乳启动和子宫恢复。
Abstract:Objective To explore the effect of early mother-to-child skin contact on newborn breastfeeding and blood glucose.Methods Eighty parturients and newborns who gave birth in the Obstetrics Department of Shenzhen Futian Maternal and Child Health Hospital from April to November 2019 were selected as the research subjects.They were divided into a control group (n=40,routine obstetric intervention) and an observation group (n=40,early skin contact) equally according to the simple randomization.The blood glucose,feeding status of the newborns,and the maternal lactation and uterine involution in the two groups were observed.Results After the intervention,the blood glucose level of newborns in the observation group was higher than that of the control group,and the incidence of hypoglycemia was lower than that of the control group with statistical significances (P<0.05).In the observation group,the rate of breastfeeding within 30 minutes after giving birth and the rate of breastfeeding completion within 90 minutes,the rate of exclusive breastfeeding before discharge,and the rate of exclusive breastfeeding for three months were all higher than those of the control group,and the differences were statistically significant (P<0.05).In the observation group,lactation starting time was shorter than that of the control group.The uterine fundus at 24 h and 48 h was lower than that at 2 h after delivery,but the height of uterine descending at 24 h and 48 h after delivery in the observation group was greater than that of the control group,and the differences were statistically significant (P<0.05).Conclusion Early mother-tochild skin contact can reduce the incidence of neonatal hypoglycemia,improve breastfeeding efficiency,and promote lactation initiation and uterine recovery.
