下肢血管彩超联合踝臂指数在老年下肢血管疾病早期诊断中的应用价值

摘要

目的探讨彩超联合踝臂指数在诊断老年人下肢血管疾病中的应用价值，为临床诊断提供参考。方法选取2016年1月～2017年3月在我院就诊的60例（观察组）老年下肢血管疾病患者和45例（对照组）同期健康体检者作为研究对象，采用彩色多普勒超声诊断仪行超声检查和全自动动脉硬化检测仪测量踝臂指数（ABI），比较两组患者彩超检查、ABI测量结果及不同方法下肢动脉病变（PAD）检出率。结果观察组患者彩超检查内膜毛糙、测量血管壁内-中膜厚度（IMT）增厚、粥样斑块形成、血流充盈缺损、管腔狭窄、管腔闭塞均明显高于对照组，差异有统计学意义（P＜0.05）；观察组患者ABI≤0.9检出率明显高于对照组，0.9＜ABI≤1.3检出率明显低于对照组，差异有统计学意义（P＜0.05）；彩超联合ABI测量PAD检出率为88.3%（53/60），明显高于单用彩超检查的66.7%（40/60）和ABI测量的41.7%（25/60），差异有统计学意义（P＜0.05）。结论采用彩超联合踝臂指数诊断老年下肢血管疾病有助于准确诊断下肢血管病变，值得在临床上推广应用。

Abstract：ObjectiveTo explore the effectiveness of application of color Doppler ultrasound combined with ankle brachial index (ABI)in the diagnosis of lower extremity vascular diseases in elderly patients,so as to provide reference for clinical diagnosis.MethodsAltogether 60 cases of elderly patients with lower extremity vascular disease who were treated in our hospital from January 2016 to March 2017 and 45 healthy cases who were underwent health examinations during the same period of time were selected as the object of this research;ultrasonic inspection with color Doppler ultrasonic diagnostic apparatus and ABI measurement with full-automatic arteriosclerosis detector were performed for all the subjects;the color Doppler ultrasound examination and ABI measurement results and peripheral arterial disease(PAD)detection rate acquired by using different methods were compared between two groups of patients.ResultsThe rough intima,IMT (Intima-Media Thickness)thickening,plaque formation,blood filling defect,lumen stenosis and lumen occlusion of the observation group examined by using ultrasound were significantly higher than those of control group,with statistically significant difference (P＜0.05);the detection rate of patients with ABI≤0.9 of the observation group was significantly higher than that of control group;and the detection rate of patients with 0.9＜ABI≤1.3 of the observation group was significantly lower than that of control group,with statistically significant difference (P＜0.05);the detection rate of PAD by using the combination of color Doppler ultrasound and ABI measurement was 88.3%(53/60),significantly higher than that of single color Doppler ultrasound examination (66.7%,40/60)and single ABI(41.7%,25/60),with statistically significant difference(P＜0.05).ConclusionApplication of color Doppler ultrasound combined with ankle brachial index in the diagnosis of lower extremity vascular disease in elderly patients is helpful for the accurate diagnosis of lower extremity vascular disease,which is worthy of clinical application.
