正清风痛宁定向透药治疗早期2型糖尿病肾病的效果

摘要 [摘要]目的探讨正清风痛宁定向透药治疗早期2型糖尿病肾病的效果。方法选取2018年4月～2019年3月我院肾病内分泌科收治的60例早期2型糖尿病肾病患者作为研究对象，按照治疗先后顺序随机将其分为对照组（30例）和治疗组（30例）。对照组给予糖尿病肾病基础治疗，治疗组在对照组的基础上配合正清风痛宁定向透药治疗，治疗4周。比较两组患者治疗前后的尿清蛋白排泄率（UAER）、24h尿蛋白定量、尿β2 微球蛋白（β2-MG）、血清清蛋白（ALB）、前清蛋白（PA）、糖化血红蛋白（HbA1c）水平，比较两组患者的治疗总有效率及不良反应发生情况。结果两组患者治疗后的UAER、24h尿蛋白定量、β2-MG、HbA1c 水平均低于治疗前，ALB、PA 均高于治疗前，差异有统计学意义（P＜0.05）；治疗后，治疗组患者的UAER、24h尿蛋白定量、β2-MG 均低于对照组，ALB、PA 均高于对照组，差异有统计学意义（P＜0.05）；两组患者治疗后的HbA1c 比较，差异无统计学意义（P＞0.05）。治疗组患者的治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。对照组患者出现1例轻度皮疹，给予抗过敏、止痒等处理后继续完成实验，观察组患者未见明显不良反应。结论正清风痛宁定向透药具有明显减少早期2型糖尿病肾病患者的尿蛋白含量、改善肾小管功能、提高患者营养指标等作用，值得在临床中应用推广。

Abstract：[Abstract]Objective To explore the effect of Zhengqing Fengtongning oriented permeation on early type 2 diabetic nephropathy.Methods From April 2018 to March 2019，60 patients with early type 2 diabetic nephropathy admitted to the nephrotic endocrinology department of our hospital were selected as research objects，and randomly divided into control group（30 cases）and treatment group（30 cases）according to the treatment sequence.Patients in the control group were given basic treatment for diabetic nephropathy，while patients in the treatment group were treated with Zhengqing Fengtongning oriented permeation therapy on the basis of the control group for 4 weeks.The urinary albumin excretion rate（UAER），24h urine protein quantification，urinary β2 microglobulin（β2-MG），serum albumin（ALB），prealbumin（PA），glycosylated hemoglobin（HbA1c）levels before and after treatment were compared between the two groups，and the total effective rate and adverse reactions of the two groups were compared.Results The UAER，24h urine protein，β2-MG，HbA1c levels of the two groups after treatment were lower than before treatment，ALB，PA were higher than before treatment，the differences were statistically significant（P＜0.05）.After treatment，UAER，24h urine protein and β2-MG of patients in the treatment group were lower than those in the control group，ALB and PA were higher than those in the control group，the differences were statistically significant（P＜0.05）.There was no significant difference in HbA1c between the two groups after treatment（P＞0.05）.The total effective rate of the treatment group was higher than that of the control group，and the difference was statistically significant（P＜0.05）.One case of mild rash occurred in the control group，and the experiment was continued after anti-allergy and antipruritic treatment.No obvious adverse reactions were found in the observation group.Conclusion Zhengqing Fengtongning oriented permeation has the effects of obviously reducing urine protein content of early type 2 diabetic nephropathy patients，improving renal tubular function，improving nutritional indicators of patients，etc.，and is worthy of clinical application and promotion.
