Experimental study on simvastatin in prevention of steroid -induced femoral head necrosis

Abstract: Objective To investigate the prevention effects of simvastatin against steroid-induced femoral head necrosis and to discuss its mechanism. Methods 36 New Zealand white rabbits were randomly divided into three groups,each with 12 rabbits.Rabbits in group A were intramuscularly injected by dexamethasone 2.5 mg/(kg·d),rabbits in group B were orally given simvastatin 20 mg/d plus intramuscularly injected by dexamethasone 2.5 mg/(kg·d),and rabbits in group C were orally given simvastatin 20 mg/d.The change of blood lipid was detected at the 12thweeks and histological observation of femoral head was conducted at the 12thweeks when the rabbits were executed. Results Compared with group B and group C,the differences of serum cholesterol,triacylglycerol,and NO level of rabbits in group A was statistically significant respectively(P<0.05).There was no significant difference between group B and group C(P>0.05).As to the level of bone metabolic markers including NO,osteocalcin(OCN),and tartrate resistant acid phosphatase(TRAP-5b), there was statistically significant difference between group A and group B(P<0.01),and between group A and group C (P<0.01), but no significant difference between group B and group C(P>0.05).Histological observation showed osteonecrosis and trabecular destruction in group A,which was not observed in group B or group C. Conclusion Simvastatin can regulate the lipid and bone metabolism,lower the blood lipid,inhibit the destruction of bone cells,and prevent femoral head necrosis.
