简化肺栓塞严重性指数及联合检测的生化标志物在肺栓塞严重性及预后评价中的价值

摘要目的探讨简化肺栓塞严重性指数（sPESI）及联合检测的生化标志物在肺栓塞严重性及预后评价中的价值。方法选取2011年3月～2013年3月在我院进行治疗的肺栓塞患者154例，筛选其中主要终点事件和次要终点事件，应用sPESI对患者进行评估、检测患者血液生化指标和影像学，探讨各种指标在肺栓塞严重性及预后评价中的价值。结果sPESI、B型钠尿肽≥514 pg/ml、超敏肌钙蛋白I≥0.1 μg/L为主要终点事件的影响因素，sPESI为次要终点事件的影响因素。结论在肺栓塞短期预后评价中，sPESI、B型尿钠肽、超敏肌钙蛋白I是优秀的评价指标，能反映肺栓塞整体风险；在肺栓塞长期预后评价中，sPESI具有较高的价值，可被融合进肺栓塞的综合风险评价中。

Abstract：Objective To discuss the evaluation values of simplified pulmonary embolism severity index combined with biochemical markers on pulmonary embolism severity and prognosis.Methods 154 patients with pulmonary embolism treated in our hospital from March 2011 to March 2013 were selected，and primary endpoints and secondary endpoints were sieved.The severity index of applying simplified pulmonary embolism was evaluated，and blood biochemical parameters and images were tested in order to explore values of each index on the pulmonary embolism severity and prognosis.Results sPESI，BNP≥514 pg/ml，and hs-TnI≥0.1 μg/L was influence factor of primary endpoints respectively，and only sPESI was considered as an influence factor of secondary outcome event.Conclusion In short-term prognosis evaluation of pulmonary embolism，sPESI，BNP，and hs-TnI are excellent evaluation indexes，and can reflect the overall risk of pulmonary embolism.However，in long-term prognosis evaluation，sPESI is the only one at a higher value and can be integrated into a comprehensive risk evaluation of pulmonary embolism.
