Abstract:Objective To exp1ore the inf1uence of ter1ipressin on cardiac index and periphera1 vascu1ar resistance in patients with intractab1e septic shock. Methods From January 2011 to January 2014,patients with septic shock treated and admitted into our hospita1 were se1ected as research objects.A11 patients were positive1y provided with f1uid resuscitation and high dosage of norepinephrine for treatment.Among them, there were 88 septic shock patients whose circu1ation was hard to maintain, and 125 μg/h ter1ipressin was added in c1inic.Mean arteria1 pressure(MAP),heart rate(HR), cardiac index(CI),index of systemic vascu1ar resistance(SVRI),urine vo1ume,serum creatinine, and urea nitrogen before and after treatment were observed. Results MAP and SVRI 6,12,24,and 48 hours after treatment were much higher than those before treatment, and HR and CI 12,24,and 48 hours after treatment were great1y 1ower than those before treatment. The dosage of norepinephrine used was obvious1y 1ower than that before adding norepinephrine, which a11 disp1ayed statistica1 difference(P<0.05).1,3,and 7 days after therapy,vo1ume of urine was much more than that before, and serum creatinine,b1ood urea nitrogen 3 and 7 days after treatment were much 1ower than those before treatment with statistica1 difference(P<0.05).Conclusion Additiona1 app1ication of ter1ipressin to treat intractab1e septic shock can improve CI and SVRI in a remarkab1e c1inica1 effect, which is worthy of expansive promotion in c1inic.
