重组人脑利钠肽联合低分子肝素钙治疗重症肺心病心力衰竭的效果

摘要目的探讨重组人脑利钠肽联合低分子肝素钙治疗重症肺心病心力衰竭的效果。方法分析番禺区中心医院2015年6月～2016年6月收治的88例重症肺心病心力衰竭患者的临床资料，依据治疗措施不同进行分组，对照组（硝酸甘油治疗）44例和联合组（重组人脑利钠肽联合低分子肝素钙治疗）44例。观察两组患者治疗前后的左室射血分数（LVEF）、左室舒张末期内径（LVDd）、脑利钠肽（BNP）、C反应蛋白（CRP）、血清肌酐（SCr）情况，观察两组患者的总有效率。结果治疗后，对照组和联合组患者的LVEF[（38.1±3.2）%、（42.7±3.5）%]高于治疗前，LVDd [（61.2±3.1）、（55.2±3.4）mm]、BNP[（1687.5±109.8）、（1050.4±99.8）ng/L]、[CRP（17.8±4.0）、（13.2±3.7）mg/L]、SCr[（122.9± 29.8）、（104.1±26.2）mmol/L]均低于治疗前，联合组患者治疗后的LVEF高于对照组，LVDd、BNP、CRP、SCr均低于对照组，联合组患者的临床治疗总有效率为97.7%，高于对照组的75.0%，差异有统计学意义（P<0.05）。结论重组人脑利钠肽联合低分子肝素钙治疗肺心病心力衰竭，可明显改善心功能，疗效良好，值得临床推广应用。

Abstract：ObjectiveTo explore the effect of Recombinant Human Brain Natriuretic Peptide combined with Low Molecular Weight Heparin Calcium in the treatment of severe pulmonary heart disease with heart failure.MethodsClinical data of 88 cases with severe pulmonary heart disease with heart failure treated in Panyu District Central Hospital from June 2015 to June 2016 were analyzed.Patients were divided into the control group(Nitroglycerin treatment,n= 44)and the combined group (Recombinant Human Brain Natriuretic Peptide combined with Low Molecular Weight Heparin Calcium treatment,n=44)according to different treatment methods.Left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVDd),brain natriuretic peptide(BNP)and C reactive protein(CRP),serum creatinine (SCr)before and after treatment in two groups were observed,and the total effective rate in two patients were observed.ResultsAfter treatment,LVEF [(38.1±3.2)%and (42.7±3.5)%]of the control group and combined group were higher than before treatment，LVDd[(61.2±3.1)mm and(55.2±3.4)mm],BNP[(1687.5±109.8)ng/L and(1050.4±99.8)ng/L], CRP[(17.8±4.0)mg/L and(13.2±3.7)mg/L],SCr[(122.9±29.8)mmol/L and(104.1±26.2)mmol/L]were lower than before treatment;LVEF of combined group after treatment was higher than that of control group,LVDd,BNP,CRP,SCr were lower than those of control group;the total effective ratein the combined group(97.7%)was higher than that of the control group (75.0%),and the differences were statistical significance (P<0.05).ConclusionRecombinant Human Brain Natriuretic Peptide combined with Low Molecular Weight Heparin Calcium in the treatment of severe pulmonary heart disease with heart failure can improve obviously the cardiac function,and the curative effect is good.It is worthy of clinical promotion and application.
