Abstract:Objective To investigate the risk factors of postoperative pancreatitis in patients with undergoing retrograde cholangiopancreatography (ERCP).Methods From January 2012 to December 2016,296 patients with bile duct stones diagnosed by ERCP were selected as the subjects,and the risk factors of postoperative pancreatitis (PEP) were analyzed from two aspects of clinical pathology and technical operation.Results In 296 cases there were 25 cases of PEP,the incidence rate was 8.4%.The single factor analysis of JPD,visualization of pancreatic duct,difficult intubation and long time operation could increase the incidence rate of PEP,with significant difference (P<0.01),had induced effect on PEP.Nasobiliary drainage or bile/pancreatic stent could reduce the incidence rate of PEP,with significant difference (P<0.01).Incision of elderly and endoscopic sphincterotomy (EST) had no effect on PEP,without significant difference (P>0.05).Logistic regression analysis showed that JPD,multiple pancreatic duct development and multiple pancreatic duct intubation were the risk factors for PEP,and nasal biliary drainage or biliary/pancreatic stent drainage were the protective factors of PEP.Conclusion Patients with JPD are at high risk for PEP,pancreatic duct development and difficult intubation are the risk factors for PEP.Long operation time should also be vigilant.Nasal biliary drainage or biliary/pancreatic stent drainage is an effective way to prevent PEP.
