时机理论对早期血液透析患者自我护理能力及生存质量的影响

摘要目的探讨以时机理论为指导护理干预对早期血液透析（HD）患者自我护理能力及生存质量的影响。方法选取2018年10月～2019年6月在我院HD 治疗的90 例终末期肾病患者作为研究对象，按照随机数字表法分为对照组（n=45）和观察组（n=45）。对照组采用常规护理干预方法，观察组采用以时机理论为指导护理干预方法，比较两组干预前后自我护理能力、并发症总发生率及生存质量。结果两组干预后自我护理能力评分均高于干预前，差异有统计学意义（P＜0.05）；干预前两组自我护理能力评分比较，差异无统计学意义（P＞0.05），干预后观察组自我护理能力评分及生存质量各维度的评分均高于对照组，且并发症总发生率低于对照组，差异有统计学意义（P＞0.05）。结论以时机理论为指导的护理干预可有效提升早期HD 患者自我护理能力，降低并发症的发生率，提高生存质量。

Abstract：Objective To explore the effect of nursing intervention guided by timing theory on self-care ability and quality of life of early hemodialysis (HD) patients.Methods A total of 90 patients with end-stage renal disease undergoing HD in our hospital from October 2018 to June 2019 were selected as the study objects.According to the random number table method,the patients were divided into control group(n=45)and intervention group(n=45).The control group adopted the routine nursing intervention method, and the intervention group adopted the nursing intervention method guided by the timing theory.The self-care ability before and after the intervention, the total incidence of complications and the quality of life of the patients in the two groups were compared.Results The scores of self-care ability in the two groups were higher than those before the intervention, the differences were statistically significant (P＜0.05).Before the intervention, there was no significant difference between the two groups in the score of self-care ability (P＞0.05).After the intervention, the scores of self-care ability and quality of life in the intervention group were higher than those in the control group, and the total incidence of complications was lower than that in the control group, the differences were statistically significant (P＜0.05).Conclusion The nursing intervention guided by the timing theory can effectively improve the self-care ability of early HD patients, reduce the incidence of complications, and improve the quality of life.
