Abstract:Objective To investigate the effect of intravascular interventional therapy in rupture and hemorrhage of middle cerebral artery aneurysm combined with hematoma. Methods Retrospective analysis of the clinical data of 33 patients with aneurysm admitted to the Fujian Provincial Hospital South Branch from June 2015 to December 2019. According to the different treatment methods, they were divided into the intervention group (18 cases) and the craniotomy group (15 cases). The interventional group received endovascular embolization, and the craniotomy group received craniotomy. The operation time, prognosis and postoperative complications of the two groups were compared. Results The operation time of the intervention group was shorter than that of the craniotomy group, the difference was statistically significant (P<0.05). There was no significant difference in Glasgow outcome scale (GOS) score of the two groups after 12 months of follow-up (P>0.05). There was no significant difference in the incidence of hydrocephalus and cerebral infarction between the two groups (P>0.05). The incidence of intracranial infection in the intervention group was lower than that in the craniotomy group, and the difference was statistically significant (P<0.05). Conclusion There is no significant difference in the long-term prognosis between intravascular interventional therapy and craniotomy in the treatment of rupture and hemorrhage of middle cerebral artery aneurysm combined with hematoma. The former has shorter operation time and lower incidence of postoperative intracranial infection. Endovascular intervention is a safe and effective surgical method for such patients.
