高脂血症急性胰腺炎的研究进展

摘要随着人们生活水平的提高和饮食方式的改变，高脂血症急性胰腺炎（HLAP）的发病率持续上升，因其具有年轻化、重症化、易复发、症状不典型等发病特点，且目前尚无相关指南对其进行指导治疗，因此HLAP正在受到越来越多的关注。本文主要从病因、发病机制、治疗等方面对国内外HLAP的相关研究进行总结，认为高脂血症是HLAP的关键诱发因素，发病机制主要与游离脂肪酸损伤、微循环障碍、炎症反应、钙超载、氧化应激反应等相关，降脂为HLAP的治疗核心，目前主要通过降脂药物、胰岛素、低分子肝素、血液净化、中医药等进行降脂治疗，然而缺乏明确的诊疗方案及病情的重症化给治疗带来了难度，故本文通过对近年来HLAP的最新研究进展作一综述，以期为临床治疗HLAP提供参考。

Abstract：With the improvement of people′s living standards and changes in dietary patterns,the incidence of hyperlipidemia acute pancreatitis (HLAP)continues to rise.It has the characteristics of younger,more severe,easy to relapse,and atypical symptoms,and there are no relevant guidelines for HLAP to guide the treatment of HLAP,so HLAP is attracting more and more attention.In this paper,we summarize the research on HLAP at home and abroad in terms of etiology,pathogenesis and treatment,and conclude that hyperlipidemia is the key predisposing factor of HLAP,and the pathogenesis is mainly related to free fatty acid damage,microcirculatory disorders,inflammatory response,calcium overload and oxidative stress and so on.However,the lack of a clear treatment plan and the severity of the disease make the treatment difficult.Therefore,the latest research on HLAP in recent years is discussed in order to provide reference for the clinical treatment of HLAP.
