区域穿刺法联合微信平台管理在肿瘤患者PICC置管期间的应用效果

摘要目的探讨区域穿刺法（ZIM）联合微信平台管理在肿瘤患者PICC置管期的应用效果。方法选取萍乡市人民医院2019年6月—2020年7月收治的90 例肿瘤患者作为研究对象，按照随机数字表法分为A 组（30 例）、B组（30 例）与C 组（30 例）。A 组采用ZIM 行B 超引导下PICC 穿刺置管术联合常规护理干预，B 组采用常规超声引导下PICC 置管术联合常规护理干预，C 组采用ZIM 行超声引导下PICC 置管术联合微信平台管理，三组均连续干预至拔管后。比较三组术后并发症的发生情况、护理满意度与自我护理能力。结果 A 组并发症总发生率低于B 组，B 组并发症总发生率高于C 组，A 组并发症总发生率高于C 组，差异均有统计学意义（P＜0.05）；出院前与拔管后A 组的护理总满意度高于B 组，B 组的护理总满意度低于C 组，A 组的护理总满意度低于C 组，差异均有统计学意义（P＜0.05）；干预前三组的自我护理能力测定量表（ESCA）各项评分比较，差异无统计学意义（P＞0.05）；与干预前比较，三组干预后ESCA 各项评分均升高，且A 组的ESCA 各项评分高于B 组，而B 组的ESCA 各项评分低于C 组，A 组的ESCA 各项评分低于C 组，差异均有统计学意义（P＞0.05）。结论 ZIM 联合微信平台管理可有效减少肿瘤患者PICC置管期并发症的发生，并能提高其自我护理能力。

Abstract：Objective To explore the application effect of zone puncture method (ZIM) combined with WeChat platform management during PICC catheterization of tumor patients.Methods A total of 90 tumor patients admitted to Pingxiang People ′s Hospital from June 2019 to July 2020 were selected as the research objects, and were divided into group A(30 cases), group B (30 cases) and group C (30 cases) according to the random number table method.Group A used ZIM guided PICC catheter placement combined with conventional nursing intervention, group B used conventional ultrasound guided PICC placement combined with conventional nursing intervention, group C used ZIM ultrasound guided PICC placement combined with WeChat platform management, all three groups continuously intervened until after extubation.The incidence of postoperative complications, nursing satisfaction rate and self-care ability of the three groups were compared.Results The total incidence of complications in group A was lower than that in group B, the total incidence of complications in group B was higher than that in group C, and the total incidence of complications in group A was higher than that in group C, the differences were statistically significant (P＜0.05).The care total satisfaction of group A before discharge and after extubation was higher than that of group B, the care total satisfaction of group B was lower than that of group C, and the care total satisfaction of group A was lower than that of group C, the differences were statistically significant (P＜0.05).There was no statistically significant difference in the exercise of self-care agency (ESCA) scores of the three groups before the intervention (P＞0.05);compared with before the intervention, the ESCA scores of the three groups increased after the intervention, and the ESCA scores of group A were higher than that of group B，the ESCA scores of group B were lower than those of group C，and the ESCA scores of group A were lower than those of group C, the differences were statistically significant (P＜0.05).Conclusion ZIM combined with WeChat platform management can effectively reduce the occurrence of complications during PICC catheterization of tumor patients and improve their self-care ability.
