腹腔镜高位宫骶韧带悬吊术治疗子宫脱垂的临床效果

摘要目的探讨腹腔镜高位宫骶韧带悬吊术治疗子宫脱垂的临床效果。方法选取2018年10月—2020年7月阿克苏地区第一人民医院妇科收治的88 例子宫脱垂患者作为研究对象，按照随机抽签法分为对照组（44 例）与试验组（44 例），对照组采用腹腔镜辅助阴道联合全子宫切除术及阴道前后壁修补术，试验组采用腹腔镜高位宫骶韧带悬吊术。比较两组的临床疗效、手术指标、性功能及生活质量。结果试验组的治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。两组的手术用时、术中出血量、住院时间比较，差异无统计学意义（P＞0.05）。术后，试验组的盆腔脏器脱垂及尿失禁性功能量表（PISQ-12）评分及生活质量量表（QOL）评分均高于对照组，差异有统计学意义（P＜0.05）。结论应用腹腔镜高位宫骶韧带悬吊术治疗子宫脱垂的临床疗效确切，可改善性功能，提高生活质量。

Abstract：Objective To investigate the clinical effect of laparoscopic suspension of high uterosacral ligament for uterine prolapse.Methods A total of 88 patients with uterine prolapse admitted to the Department of Gynecology, the First People′s Hospital of Aksu Prefecture from October 2018 to July 2020 were selected as the research subjects and divided into control group (44 cases) and experimental group (44 cases) according to random drawing method.The control group used laparoscopic assisted vaginal hysterectomy and vaginal anterior and posterior wall repair, and the experimental group used laparoscopic high uterosacral ligament suspension.Clinical efficacy, surgical indexes, sexual function and quality of life were compared between the two groups.Results The total effective rate of experimental group was higher than that of control group, and the difference was statistically significant (P＜0.05).There was no significant difference in operation time, intraoperative blood loss and hospital stay between the two groups (P＞0.05).Postoperatively, the scores of pelvic organ prolapse and urinary incontinence sexual function scale (PISQ-12) and quality of life(QOL) of pelvic organ prolapse and urinary incontinence in experimental group were higher than those in control group,the differences were statistically significant (P＜0.05).Conclusion Laparoscopic suspension of high uterosacral ligaments is effective in the treatment of uterine prolapse, which can improve sexual function and quality of life
