Abstract:Objective To observe the effect of early pulmonary rehabilitation grading nursing scheme on the enhanced recovery in intensive care unit(ICU)of patients undergoing cardiac surgery.Methods A total of 60 patients scheduled for cardiac surgery in ICU from June 2018 to June 2020 in North Hospital of Huashan Hospital Affiliated to Fudan University were selected and divided into the control group(n=30)and the test group(n=30)by random number table method.The control group adopted routine nursing scheme and the test group adopted early pulmonary rehabilitation grading nursing scheme on the basis of routine nursing scheme.The operation condition(operation duration,intraoperative bleeding volume,intraoperative fluid infusion volume,intraoperative urine volume),lung ventilation indexes(oxygenation index,arterial partial pressure of oxygen[PaO2],arterial partial pressure of carbon dioxide[PaCO2],lactic acid value),pneumonia-related inflammation indexes(C reactive protein[CRP],procalcitonin[PCT],white blood cell count[WBC])before and after nursing intervention were compared.Further,the incidence of complications,ICU duration and nursing satisfaction of patients were recorded.Results There were no statistically significant in operation duration,intraoperative bleeding volume,intraoperative fluid infusion volume and intraoperative urine volume between two groups(P>0.05).After nursing intervention,the oxygenation index,PaO2 of test group were higher than those of the control group,PaCO2 and lactic acid value lower than those of the control group,the differences were statistically significant(P<0.05).Also,the serum CRP,PCT,WBC levels in test group were lower than those of the control group after nursing intervention,the differences were statistically significant(P<0.05).Moreover,ICU duration time of the test group was shorter than that of the control group,complications incidences was lower than that of the control group,satisfaction in nursing was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusions Early pulmonary rehabilitation grading nursing scheme can significantly improve the respiratory function and prognosis and reduce the occurrence of postoperative complications of cardiac surgery patients during ICU,which is safer and more effective.
