Clinical effect of endotracheal intubation under the guidance of fiberoptic bronchoscope in the treatment of acute respiratory failure

摘要目的 观察纤支镜引导下经鼻气管插管治疗急性呼吸衰竭的临床效果。方法 选取2017年1月～2019年12月阳江市中医医院收治的46例急性呼吸衰竭患者作为研究对象。按照随机数字表法分对照组和观察组，每组各23例。对照组给予经口气管插管治疗，观察组给予纤支镜引导下经鼻气管插管治疗。比较两组患者插管前后血气指标以及并发症总发生率。结果 两组患者插管前的各项血气指标比较，差异无统计学意义（P＞0.05）；观察组患者插管后24、72 h 动脉血氧分压（PaO2）高于对照组，观察组插管后12、24、72 h 动脉血二氧化碳分压（PaCO2）低于对照组，差异有统计学意义（P＜0.05）；观察组患者并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。结论 纤支镜引导下经鼻气管插管治疗急性呼吸衰竭，对患者血气指标改善作用显著，并发症少，可予以推广。
Abstract：Objective To observe the clinical effect of endotracheal intubation under the guidance of fiberoptic bronchoscope in the treatment of acute respiratory failure.Methods A total of 46 patients with acute respiratory failure admitted to Yangjiang Hospital of Traditional Chinese Medicine from January 2017 to December 2019 were selected as the research subjects.The patients were divided into the control group and the observation group according to the random number table method,with 23 cases in each group.The control group was treated with oral endotracheal intubation,and the observation group was treated with nasal endotracheal intubation under the guidance of fiberoptic bronchoscope.The blood gas indexes and the total incidence of complications before and after intubation were compared between the two groups.Results There were no significant differences in various blood gas indexes before intubation between the two groups (P＞0.05).The partial pressures of arterial oxygen (PaO2) at 24 and 72 hours after intubation in the observation group were higher than those in the control group,and the partial pressures of arterial carbon dioxide (Pa-CO2) at 12,24 and 72 hours after intubation in the observation group was lower than that in the control group at 12,24 and 72 hours after intubation,the differences were statistically significant (P＜0.05).The total incidence of complications in observation group was lower than that in control group,the difference was statistically significant (P＜0.05).Conclusion The treatment of acute respiratory failure by nasotracheal intubation under the guidance of fibrobronchoscope can significantly improve the blood gas index of patients with less complications,which can be popularized.
