Study on risk factors and prognosis of nosocomial infection in elderly patients with chronic gastritis complicated with upper gastrointestinal bleeding

Abstract:Objective To study on risk factors and prognosis of nosocomial infection in elderly patients with chronic gastritis complicated with upper gastrointestinal bleeding(UGIB).Methods A total of 358 elderly patients with chronic gastritis admitted to the Department of Gastroenterology,Zhuanghe Central Hospital from October 2018 to January 2020 were selected as the research subjects.Among them,a total of 173 patients with UGIB were included in the study group,and the remaining a total of 185 patients without UGIB were included in the control group.The incidence of infection and prognosis during hospitalization were compared between the two groups of patients,and the logistic regression equation was used to analyze the influencing factors of the patient′s infection.Results The incidence of infection in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in gender,body mass index,hypertension,lung disease,blood transfusion treatment and chronic gastritis between infected and uninfected patients(P>0.05);age of infected people,prevalence of diabetes,invasive hemostasis application the number of cases were higher than those of uninfected patients,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that age≥70 years,long hospital stay,diabetes mellitus and invasive hemostasis were all risk factors for nosocomial infection in elderly patients.According to the bleeding volume of the patients,a total of 60 cases of mild bleeding patients,a total of 75 cases of moderate bleeding patients,and 38 cases of severe bleeding patients were shown.The recurrence rate,fatality rate and infection rate of severe bleeding patients were higher than those of moderate and mild bleeding patients,the differences were statistically significant(P<0.05).Conclusion The elderly patients with chronic gastritis and UGIB are at higher risk of nosocomial infections.Among them,patients≥70 years of age,longer hospital stay,diabetes and invasive hemostasis are all risk factors for nosocomial infection,and patients with heavier bleeding the higher incidence of infection and the worse the prognosis.
