超声造影联合肾动态显像在早期慢性肾功能不全中运用

摘要目的研究超声造影（CEUS）联合肾动态显像在早期慢性肾功能不全（CRF）中的应用效果。方法以2019年9月—2020年9月于南昌市第一医院接受治疗的68 例早期CRF 患者为病例组，选取同期的40 例体检健康志愿者作为对照组，对所有受检者的临床资料进行研究，比较两组的时间-强度曲线（TIC）的上升支斜率参数（A）、下降支斜率参数（α）、峰值强度（DPI）、曲线下面积（AUCTIC）、到峰时间（TTP）和双肾肾血流灌注指数（PI）、肾小球滤过率（GFR），使用受试者工作特征（ROC）曲线评估上述指标定量诊断早期CRF 的诊断效能。结果病例组的DPI小于对照组（P＜0.05），A、AUCTIC、双肾PI 大于对照组（P＜0.05），TTP 长于对照组（P＜0.05）。两组的α、GFR 比较，差异无统计学意义（P＞0.05）；CEUS 联合肾动态显像诊断的ROC 曲线AUC 为0.968（95%CI=0.943～0.994），敏感度为91.20%，特异度为90.00%。结论 CEUS 联合肾动态显像定量参数早期诊断CRF 具有较高准确度，可为诊治决策提供更客观准确的临床依据。

Abstract：Objective To study the application effect of contrast-enhanced ultrasound(CEUS)combined with renal dynamic imaging in early chronic renal failure(CRF).Methods A total of 68 patients with early CRF who were treated in the First Hospital of Nanchang City from September 2019 to September 2020 were selected as the case group,and 40 healthy volunteers who had physical examination in the same period were selected as the control group.The clinical data of all subjects were studied,and the ascending branch slope parameter(A),descending branch slope parameter(α),derived peak intensity(DPI),area under the curve(AUCTIC)of time-intensity curve(TIC),time to peak(TTP),renal blood perfusion index(PI)and glomerular filtration rate(GFR)of both kidneys were compared between the two groups.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of the above indicators in the quantitative diagnosis of early CRF.Results The DPI of the case group was lower than that of the control group(P＜0.05),and the A,AUCTIC and PI of both kidneys of the case group were greater than those of the control group(P＜0.05),and the TTP of the case group was longer than that of control group(P＜0.05).There were no significant differences in α and GFR between the two groups(P＞0.05).The AUC of ROC curve,sensitivity and specificity of CEUS combined with renal dynamic imaging were 0.968(95%CI=0.943-0.994),91.20% and 90.00%.Conclusion CEUS combined with quantitative parameters of renal dynamic imaging has high accuracy in the early diagnosis of CRF,which can provide a more objective and accurate clinical basis for diagnosis and treatment decisions.
