不同神经阻滞方法在锁骨骨折患者内固定术中的应用效果

摘要目的探讨不同神经阻滞方法在锁骨骨折患者内固定术中的应用效果。方法选取我院2015年1月～2017年1月收治的80例锁骨骨折内固定术患者作为研究对象，随机分为对照组和观察组，各40例。对照组采用单纯颈从阻滞麻醉，观察组采用颈丛臂丛联合阻滞麻醉，比较两组的麻醉效果及并发症情况。结果两组麻醉前的心率及NIBP比较，差异无统计学意义（P＞0.05）。观察组切片时、骨折复位时的心率及NIBP高于对照组，差异有统计学意义（P＜0.05）。两组术后24 h的心率比较，差异无统计学意义（P＞0.05）。观察组术后24 h的NIBP低于对照组，差异有统计学意义（P＜0.05）。观察组患者的麻醉优良率明显高于对照组，差异有统计学意义（P＜0.05）。观察组患者的术后并发症发生率明显低于对照组，差异有统计学意义（P＜0.05）。结论对锁骨骨折患者内固定术实施颈丛臂丛联合阻滞麻醉效果显著，有利于改善患者麻醉效果，减少神经损伤程度，临床价值显著，值得推广应用。

Abstract：ObjectiveTo explore the application effect of different nerve block methods in the internal fixation of patients with clavicle fractures.MethodsFrom January 2015 to January 2017,80 patients with clavicle fracture who were treated by internal fixation in our hospital were selected.The patients were evenly divided into the control group and the observation group in random,40 cases in each group.In the control group,simple neck block anesthesia was used,while in the observation group,the combined cervical and brachial plexus block anesthesia was adopted.The anesthetic effect and complications were compared between the two groups.ResultsThere was no significant difference in the heart rate and NIBP between the two groups before anesthesia (P＞0.05).The heart rate and NIBP during slicing and reduction of fracture in the observation group were higher than those in the control group,with significant difference (P＜0.05).There was no significant difference in the heart rate between the two groups at 24 h after operation(P＞0.05).The NIBP at 24 h after operation in the observation group was lower than that in the control group,with significant difference(P＜0.05).The excellent and good rate of anesthesia in the observation group was much better than that in the control group,with statistical difference(P＜0.05).The postoperative incidence of complications in the observation group was lower than that in the control group,which was displayed statistical significance (P＜0.05).ConclusionFor patients with clavicle fractures,application of combined cervical and brachial plexus block anesthesia for internal fixation can obtain a remarkable effect,which is beneficial to improve the anesthetic effect and alleviate the severity of nerve injury in a great clinical value.It is worthy of extensively promoted.
