枸地氯雷他定不同剂量方案治疗慢性荨麻疹的效果对比

摘要目的对比枸地氯雷他定不同剂量方案治疗慢性荨麻疹的效果。方法将我院2016年1～6月门急诊和皮肤科就诊的62例慢性荨麻疹患者按照随机数字表法分为观察组和对照组各31例，观察组患者给予枸地氯雷他定递减用药方法治疗，对照组患者给予枸地氯雷他定常规用药方法治疗。观察两组患者治疗前后临床症状评分，比较两组患者治疗效果、治疗前后血清IgE水平改变，记录两组患者不良反应及复发情况。结果两组患者治疗后瘙痒程度、风团大小、风团数目及持续时间症状评分均较治疗前显著降低（P＜0.01）；治疗后，两组临床症状比较，差异有统计学意义（P＜0.05）。观察组患者治疗总有效率为93.55%，显著高于对照组的74.19%，差异有统计学意义（P＜0.05）。治疗后两组患者IgE水平均较治疗前显著降低（P＜0.01），组间差异无统计学意义（P＞0.05）；停药后4周观察组患者IgE水平较治疗后无明显升高（P＞0.05），而对照组患者IgE水平较治疗后显著升高（P＜0.01），观察组患者IgE水平显著低于对照组（P＜0.01）。观察组患者不良反应发生率为6.45%，对照组为9.68%，差异无统计学意义（P＞0.05）。观察组患者复发率为19.35%，显著低于对照组的48.39%，差异有统计学意义（P＜0.05）。结论枸地氯雷他定递减疗法可在改善症状的同时显著提高治疗有效率，改善血清IgE水平并降低复发率。

Abstract：Objective To compare the clinical effect of different dosage regimen of Desloratadine citrate disodium in treating chronic urticaria.Methods A total of 62 patients with chronic urticaria who were treated in our hospital from Jannary to June 2016 were enrolled in and divieded into observation group and control group according to random number table method,with 31 patients in each group.Patients in the observation group were given decremental treatment of Desloratadine citrate disodium,while patients in the control group were given conventional treatment.The clinical symptom score,clinical effect,level of serum IgE,adverse reaction rate and recurrence rate of two groups were recorded and compared.Results Clinical symptom scores of pruritus,wheal size,wheal number and duration after treatment in two groups were significantly lower than those before treatment(P＜0.01);after treatment,the difference in clinical symptom score between two groups was statistically significant(P＜0.05).The total effective rate of the observation group was 93.55%,which was significantly higher than that 74.19%of the control group,and the difference was statistically significant(P＜0.05).Level of IgE after treatment in two groups was all significantly lower than that before treatment(P＜0.01), there was no statistically significant difference in level of IgE between two groups(P＞0.05);4 weeks after withdrawal, level of IgE in the observation group was not significantly higher than that after treatment(P＞0.05),while level of IgE in the control group was significantly increased than that after treatment(P＜0.01),level of IgE in the observation group was significantly lower than that in the control group(P＜0.01).Incidence rate of adverse reactions in the observation group was 6.45%,in the control group with 9.68%,and the difference was not statistically significant(P＞0.05).Recurrence rate in the observation group was 19.35%,significantly lower than 48.39%in the control group,and the difference was statistically significant(P＜0.05).Conclusion The decremental treatment of Desloratadine citrate disodium can enhance effective rate,improve level of serum IgE and reduce recurrence rate.
