重组人脑钠肽治疗慢性心力衰竭急性失代偿性重症患者的效果分析

摘要目的分析重组人脑钠肽治疗慢性心力衰竭急性失代偿性重症患者的效果。方法选取我院2015年1月～2016年1月收治的90例慢性心力衰竭急性失代偿性重症患者作为研究对象，将其随机分为观察组与对照组，每组45例患者。观察组患者给予静脉推注重组人脑钠肽进行治疗；对照组患者给予静脉推注米力农进行治疗。对比观察两组患者治疗后的总有效率；治疗后的血流动力学参数（心肌纵向SR、CI、SV）；治疗后的尿量、心率、平均动脉压、血浆BNP变化、心功能（LVEDD、LVEF）改善情况。结果观察组患者的总有效率为97.77%，对照组患者的总有效率为84.44%，两组比较，差异有统计学意义（P<0.05）；治疗后两组患者的平均动脉压及LVEDD比较，差异无统计学意义（P>0.05）；治疗后观察组患者的血流动力学参数（心肌纵向SR、CI、SV）、尿量、心率、血浆BNP变化、LVEF改善情况均优于对照组患者，差异有统计学意义（P<0.05）。结论给予慢性心力衰竭急性失代偿性重症患者静脉推注重组人脑钠肽治疗，能显著提高治疗的总有效率，改善患者的临床症状及心功能，值得临床推广应用。

Abstract：ObjectiveTo analyze the efficacy of recombinant human brain natriuretic peptide in the treatment of acute decompensated severe patients with chronic heart failure.Methods90 acute decompensated severe patients with chronic heart failure who were treated in our hospital from January 2015 to January 2016 were selected as the study subjects. They were randomly divided into the observation group and the control group,with 45 patients in each group.Patients in the observation group were given intravenous injection of recombinant human brain natriuretic peptide for treatment;patients in the control group were treated with intravenous injection of Milrinone.The total effective rate after treatment, hemodynamic parameters after treatment(myocardial longitudinal SR,CI,SV),and the volume of urine,heart rate,mean arterial pressure,plasma BNP changes,and the improvement of cardiac function(LVEDD,LVEF)were compared between the two groups.ResultsThe total effective rate was 97.77%in the observation group and 84.44%in the control group. The difference compared between the two groups was statistically significant(P<0.05);there was no statistically significant difference in mean arterial pressure and LVEDD after treatment between the two groups(P>0.05);after treatment,the hemo dynamic parameters(myocardial longitudinal SR,CI,SV),urine volume,heart rate,plasma BNP changes and LVEF improve ment in the observation group were better than those in the control group after treatment(P<0.05),and the differences were statistically significant.ConclusionThe application of intravenous injection of recombinant human brain natriuretic peptide in the treatment of acute decompensated severe patients with chronic heart failure can significantly enhance the total effective rate of treatment,improve the clinical symptoms and the cardiac function.It is worthy of clinical promotion and application.
