多巴胺联合多巴酚丁胺联合治疗小儿重症肺炎的效果和对炎症反应的影响

摘要目的分析多巴胺联合多巴酚丁胺治疗小儿重症肺炎的临床效果及对炎症反应的影响。方法收集2014年10月～2016年5月我院收治的78例重症肺炎患儿，采用随机数字表法分为观察组和对照组，每组各39例。对照组采取吸氧、抗感染、止咳平喘、纠正酸碱平衡及电解质紊乱等常规治疗，观察组在此基础上加用多巴胺联合多巴酚丁胺治疗，比较两组临床疗效及血清炎症因子水平。结果观察组有效率为94.87%（37/39），显著高于对照组的79.49%（31/39），差异有统计学意义（P＜0.05）；观察组治疗后IL-6、TNF-α和CRP水平与治疗前及对照组比较均显著降低，差异有统计学意义（P＜0.05）。结论多巴胺联合多巴酚丁胺治疗小儿重症肺炎的效果显著，可明显改善患儿的临床症状并有效抑制炎症反应，值得临床推广应用。

Abstract：ObjectiveTo analyze the effect of dopamine and dobutamine in the treatment of children with severe pneumonia and their influence on inflammatory reaction.MethodsSeventy-eight children with severe pneumonia from October 2014 to May 2016 admitted into our hospital were selected and evenly divided into two groups by a random number table,observation group and control group,with 39 cases in each group.In the control group,regular treatments including oxygen inhalation,anti-infection,cough arresting and asthma relieving,correction of acid-base balance and electrolyte disorders were used.In the observation group,on the basis of the control group,dopamine combining with dobutamine was added for the treatment.The clinical effect and serum inflammatory factor level in the two groups were compared.ResultsThe effective rate in the observation group was 94.87% (37/39),much higher than that in the control group accounting for 79.49%(31/39),the difference was statistically significant(P＜0.05).In the observation group,levels of IL-6, TNF-α,and C-reactive protein (CRP)after treatment were greatly decreased in comparison with that before treatment and that in the control group after treatment,the differences were statistically significant(P＜0.05).ConclusionDopamine combining with dobutamine can obtain a remarkable effect on treating pediatric severe pneumonia and can significantly improve the clinical symptoms in children and inhibit the inflammatory response,which is worthy of promotion and application in clinic.
