医院与社区联合进行慢性心力衰竭中西医结合专病管理模式的探索

Abstract ObjectiveTo study the clinical effect of combination of hospital and community special disease management mode of TCM and Western medicine for chronic heart failure (CHF).Methods150 patients with CHF treated in our hospital from May 2015 to May 2016 were selected and randomly divided into two groups,75 cases in each group.Patients in control group adopted the existing medical mode,patients in observation group adopted hospital and community special disease management mode of TCM and Western medicine.The clinical effect between two groups was observed.ResultsThe difference of proper exercise,low salt diet,quiting smoking and medication compliance before and after administration in two groups had statistical significance(P＜0.05);after administration,proper exercise,low salt diet,quiting smoking and medication compliance in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P＜0.05).The total effective rate of cardiac function in the observation group was 90.7%,higher than 73.3%in the control group,and the difference was statistically significant(P＜0.05).After management, QOL integral and LVEF of the observation group was significantly better than that of the control group,and the difference was statistically significant(P＜0.05).The total mortality rate,rehospitalization rate,emergency rate and deterioration rate in the observation group was significantly lower than that in the control group,and the difference was statistically significant (P＜0.05).ConclusionFor patients with CHF,hospital and community special disease management mode of TCM and Western medicine can improve the patient′s compliance,treatment effect,prognosis and life quality.It is worthy of promotion.
