脑室-腹腔分流术联用颅骨修补术治疗脑积水合并颅骨缺损的临床效果

摘要目的探讨脑室－腹腔分流术（V－P）联用颅骨修补术治疗脑积水合并颅骨缺损的临床效果。方法选择我院2014年12月～2017年4月收治的脑积水合并颅骨缺损患者60例，按照治疗方法的不同分为治疗组与对照组，各30例。治疗组患者同期开展V－P术联合颅骨修补术，对照组患者先行V－P流术、2～3个月后再接受颅骨修补术，评价两组的神经功能、机体功能及并发症的发生。结果治疗组术后15、30 d的神经功能缺损评分降幅均高于对照组。治疗组术后日常生活能力量表（ADL）、Fugl－Meyer、格拉斯哥昏迷评分（GCS）、格拉斯哥预后评分（GOS）评分增幅均优于对照组。治疗组的并发症发生率（13.3%）明显低于对照组（36.7%）。差异均有统计学意义（P＜0.05）。结论（V－P）术联用颅骨修补术治疗脑积水合并颅骨缺损患者，能减少患者的术后并发症，对患者的身体康复具有积极的意义。

Abstract：ObjectiveTo investigate the clinical effect of ventriculoperitonealV－Pshunt combined with cranioplasty in the treatment of hydrocephalus complicated with skull defect.Methods60 cases of hydrocephalus complicated with skull defect were treated in our hospital from December 2014 to April 2017.The treatment group was divided into the treatment group and the control group,and 30 cases in each group.The patients in the treatment group underwent ventriculoperitoneal shunt combined with cranioplasty,while patients in the control group underwentV－Pshunt and underwent cranioplasty after 2－3 months.The neurological function,body function and complications of the two groups were evaluated.ResultsThe scores of neurological deficits in 15 day and 30 day after treatment were higher in the treatment group than in the control group.The scores of ADL,Fugl－Meyer,GCS and GOS in the treatment group were better than those in the control group.The complication rate of the treatment group (13.3%)was significantly lower than that of the control group (36.7%).The differences were statistically significant(P＜0.05).ConclusionV－Pshunt combined with cranioplasty for hydrocephalus combined with skull defect can reduce postoperative complications and has positive significance for the rehabilitation of patients.
