全膝关节置换术联合止痛四物汤治疗膝关节骨性关节炎的效果

摘要目的探讨全膝关节置换术联合止痛四物汤治疗膝关节骨性关节炎的疗效及对患者血清C反应蛋白（CRP）水平的影响。方法选取2013年1月～2015年1月我院收治的膝关节骨性关节炎患者88例，采用随机数字表法将其分为对照组、观察组，每组44例。对照组行人工全膝关节置换术治疗，观察组在对照组基础上加用止痛四物汤进行治疗，比较两组CRP水平、IL-1水平、膝关节屈曲度、HSS评分和临床疗效。结果过治疗后，观察组治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。两组患者的CRP水平和IL-1水平均低于治疗前，观察组的CRP水平和IL-1水平均低于对照组，差异均有统计学意义（P＜0.05）。经过治疗后，两组膝关节屈曲度和HSS评分高于治疗前，观察组膝关节屈曲度和HSS评分均高于对照组，差异均有统计学意义（P＜0.05）。两组患者不良反应发生情况的比较，差异无统计学意义（P＞0.05）。结论全膝关节置换术联合止痛四物汤可有效提升膝关节骨性关节炎的治疗效果，改善患者的膝关节评分，且安全性高，值得在临床上推广应用。

Abstract：Objective To investigate the effect of total knee arthroplasty combined with Zhitong Siwu Decoction on knee osteoarthritis and its influence on serum C reactive protein(CRP)level.Methods From January 2013 to January 2015,88 patients with knee osteoarthritis in our hospital were selected and randomly divided into control group and observation group,with 44 cases in each group.The control group was treated with total knee arthroplasty,while the observation group was treated with Zhitong siwu decoction on the basis of the control group.CRP level,IL-1 level,knee flexion,HSS score and clinical efficacy were compared between the two groups.Results After treatment,the total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P＜0.05).The levels of CRP and IL-1 in the two groups were lower than those before treatment.the levels of CRP and IL-1 in the observation group were lower than those in the control group,the difference was statistically significant(P＜0.05).After treatment,the knee flexion and HSS score of the two groups were higher than those before treatment,and the knee flexion and HSS score of the observation group were higher than those of the control group,the difference was statistically significant(P＜0.05).There was no significant difference in adverse reactions between the two groups(P＞0.05).Conclusion Total knee arthroplasty combined with Zhitong Siwu Decoction can effectively improve the therapeutic effect of knee osteoarthritis,it can improve the score of knee joint,and has high safety,which is worthy of clinical application.
