社会支持对老年2型糖尿病患者自我感受负担与健康状况的影响研究

摘要目的探讨社会支持对老年2型糖尿病患者自我感受负担与健康状况的影响。方法选取2017年8～9月在济宁市中医院门诊就诊的385例老年糖尿病患者作为研究组，并同期选择385例健康人作为对照组。采用社会支持评定量表（SSRS）、自我感受负担量表（SPBS）和简明健康状况量表（SF-36）对其进行调查，比较两组的各量表评分情况。结果研究组患者的 SSRS 总分为（36.85±5.34）分，SPBS 总分为（29.44±5.25）分，SF-36 总分为（65.89±16.52）分；对照组 SSRS 总分为（43.76±7.41）分，SPBS 总分为（18.36±3.17）分，SF-36 总分为（77.75±17.35）分，研究组患者的SSRS及SF-36评分均明显低于对照组，而SPBS评分明显高于对照组，差异有统计学意义（t=14.844、35.450、9.714，P＜0.01）。 Pearson 相关性分析结果显示，社会支持与自我感受负担成负相关（r=-0.539，P＜0.01），社会支持与健康状况成正相关（r=0.473，P＜0.01），自我感受负担与健康状况成负相关（r=-0.388，P＜0.01）。双变量相关分析结果显示，自我感受负担、社会支持和健康状况三者之间均存在两两相关性。中介效应分析结果显示，社会支持的中介效应为0.315，中介效应与总效应的比值为0.454。结论社会支持是老年2型糖尿病患者自我感受负担与健康状况的影响因素。

Abstract：ObjectiveTo explore the influence on social support between self-perception burden and health condition in the elderly patients with type 2 diabetes.MethodsFrom August to September 2017,385 elderly patients with diabetes in the outpatient department of Jining hospital of traditional chinese medicine were selected as the research group,and 385 healthy people were selected as the control group during the same period.The social support rating scale(SSRS),the self-perceived burden scale(SPBS)and the simple health status scale(SF-36)were used to investigate and the scores of the two groups were compared.Results The total score of SSRS in the study group was (36.85±5.34)points,the total score of SPBS was(29.44±5.25)points,and the total score of SF-36 was(65.89±16.52)points.The total score of SSRS in the control group was(43.76±7.41)points,the total score of SPBS was(18.36±3.17)points,the total score of SF-36 was(77.75±17.35)points.The score of SSRS and SF-36 in the study group were significantly lower than that in the control group,while the SPBS score was significantly higher than that in the control group,the differences were statistically significant(t=14.844,35.450,9.714,P＜0.01).Pearson correlation analysis results showed that social support was negatively correlated with the burden of self-perception (r=-0.539,P＜0.01),social support was positively correlated with health condition (r=0.473,P＜0.01),the burden of self-perception was negatively correlated with health condition (r=0.388,P＜0.01).The result of the bivariate correlation analysis showed that there were correlation among social support,burden of self-perception and health condition.The result of the mediation effect analysis showed that the mediating effect the values of social support was 0.315,and the ratio of mediating effect to total effect was 0.454.Conclusion Social support is the influence factor of self-perception burden and health condition in the old age patients with type 2 diabetes.
