下肢静脉彩超联合Caprini评分评估慢性精神分裂症患者深静脉血栓风险的价值

Abstract Objective To explore the clinical value of lower extremity venous color Doppler ultrasound combined with Caprini score to assess the risk of deep vein thrombosis (DVT) in patients with chronic schizophrenia. Methods A total of 80 patients with chronic schizophrenia and suspected deep vein thrombosis of the lower extremities were selected from June 2019 to December 2020. According to the results of deep venography, they were divided into observation group (DVT group) and control group (non-DVT group), with 40 cases in each groups. The clinical manifestations (including lower limb pain, swelling, and color change) and indicators of coagulation function (including plasma thrombin time [TT], prothrombin time [PT], activated partial thromboplastin time [APTT], and fibrinogen [Fib]) of the two groups were compared. The ultrasound results and Caprini scores of the lower extremities were compared between the two groups. The value of the lower extremity venous color Doppler ultrasound combined with the Caprini score for DVT risk assessment in patients with chronic schizophrenia was analyzed. Results The cases of lower limb pain, swelling, and color change in the observation group were more than those in the control group, with statistically significant differences(P＜0.05). The TT, PT and APTT of the observation group were shorter than those of the control group, with statistically significant differences (P＜0.05). And the level of Fib in the observation group was higher than that in the control group,with statistically significant difference (P＜0.05). The vascular ultrasound results in the lower limbs was better than in the control group, with statistically significant difference (P＜0.05). The Caprini score of the observation group was higher than that of the control group, with statistically significant difference (P＜0.05). The sensitivity of lower limb venous color Doppler ultrasound combined with Caprini score to develop DVT diagnosis in patients with chronic schizophrenia was 97.4%, and the specificity was 50.0%. Conclusion The combination of lower extremity venous color Doppler ultrasound combined with Caprini score has high predictive sensitivity in the evaluation of lower extremity DVT in patients with chronic schizophrenia, and it has a positive significance in guiding clinical prevention and treatment.
