Comparison of screening of Group B streptococcus by different detection methods in late pregnancy

Abstract:Objective To explore the diagnostic value of bacterial cultivation,immunochromatography and real-time fluorescence quantitative PCR on Group B streptococcus in pregnant women of late pregnancy.Methods A total of 702 pregnant women in late pregnancy who underwent the obstetric examination in Yantian People′s Hospital of Shenzhen City from April 2018 to July 2019 were selected as the research objects.The vaginal-rectal secretions were collected as samples and detected by direct inoculation bacterial cultivation,immunochromatography and PCR.The samples were cultured in selective broth medium and then tested by the above three methods.The results of bacterial cultivation,immunochromatography and PCR before enrichment cultivation and the above three methods and after enrichment cultivation were collected.The sensitivity,specificity,accuracy and consistency of each test method were compared with gold standard of bacterial enrichment cultivation.Results The positive rate of bacterial cultivation before enrichment cultivation was 2.8%(20/702),and that after enrichment cultivation was 9.4%(66/702).The positive rate of immunochromatography and PCR before enrichment cultivation was 4.6%(32/702)and 8.5%(60/702).The positive rate of immunochromatography and PCR after enrichment cultivation was 13.8%(97/702)and 12.5%(88/702).The sensitivity,specificity,accuracy and consistency(Kappa)were 36.4%,98.7%,92.9% and 0.456 with immunochromatography before enrichment cultivation.The sensitivity,specificity,accuracy and consistency(Kappa)were 80.3%,93.1%,91.9% and 0.606 with immunochromatography after enrichment cultivation.The sensitivity,specificity,accuracy and consistency(Kappa)were 65.2%,97.3%,94.3% and 0.651 with PCR before enrichment cultivation.The sensitivity,specificity,accuracy and consistency(Kappa)were 87.9%,95.3%,94.6% and 0.724 with PCR after enrichment cultivation.Conclusion The positive rate of Group B streptococcus is higher in enrichment of bacteria in selective broth medium.The positive detection rate of immunochromatography and PCR is higher than that of bacterial cultivation.PCR has the advantages of high sensitivity,good consistency,short time-consuming and batch operation.
