全胸腔镜下心脏微创手术的临床研究

摘要目的探讨全胸腔镜下心脏微创手术的临床应用价值。方法选取2011年1月～2016年12月我院收治的56例先天性心脏病患者作为研究对象，按治疗方法的不同将其分为研究组与对照组，每组各28例。研究组患者实施全胸腔镜下心脏微创手术，对照组患者实施传统开胸心脏外科手术。比较两组患者的治疗效果。结果研究组患者的手术时间、体外循环转流时间、主动脉阻断时间长于对照组，术后住监护室时间、呼吸机使用时间、引流管留置时间、术后住院时间均短于对照组，差异有统计学意义（P＜0.05）；研究组患者的术后胸管引流量、输血量少于对照组，疼痛评分低于对照组，切口满意度高于对照组，差异有统计学意义（P＜0.05）。两组患者的残余漏发生率、死亡率比较，差异无统计学意义（P＞0.05）。结论全胸腔镜下心脏微创手术是安全可靠的，与传统心脏手术相比，具有创伤小、出血少、输血少、术后疼痛轻、恢复快、美容等优点，明显减少临床用血量，推广价值高。

Abstract：ObjectiveTo investigate the clinical effectiveness of minimally－invasive cardiac surgery under complete video－assisted thoracoscopy(cVATS).Methods Altogether a total of 56 patients with congenital heart disease admitted in our hospital from January 2011 to December 2016 were selected as objects of this study and divided into study group and control group according to different treatment methods,28 cases in esch group.The study group were treated with minimally－invasive cardiac surgery under cVATS,while the control group were treated with traditional thoracotomy cardiac surgery.The therapeutic effects of the two groups were compared.ResultsThe duration ofsurgery, cardiopulmonary bypass time and aortic cross－clamping time of study group were longer than those of control group, while the postoperative ICU time,ventilation time,duration of drainage and length of post－operative hospitalization of study group were shorter than those of control group,the differences were statistically significant(P＜0.05).The postoperative thoracic duct drainage and transfusion volume of study group were less than those of control group;pain scores of study group were lower than those of control group and the incision satisfaction was higher than that of control group, the differences were statistically significant(P＜0.05).The incidence of residual leakage and mortality were compared between the two groups,the difference was not statistically significant (P＞0.05).ConclusionCompared with the traditional cardiac surgery,minimally－invasive cardiac surgery under cVATS is a safe and reliable surgery with fewer traumas,less bleeding,less blood transfusion,alleviated postoperative pain,quicker recovery and other features.It significantly reduces the clinical blood volume,which has high popularization value.
