心理引导联合手摇滞留乳牙在基层医院6～10岁儿童滞留乳牙病例中的实践应用

摘要目的探讨心理引导联合手摇滞留乳牙在基层医院6～10岁儿童滞留乳牙病例中的应用价值。方法选取南昌市第四医院口腔科2016年2月～2018年2月收治的60例6～10岁儿童需要实施拔牙的患者作为研究对象，根据PEMS 3.0软件包生成的随机数字表分为参考组与联合组，每组30例。参考组单纯给予心理引导浸润麻醉拔除，联合组给予心理引导联合手摇滞留乳牙。比较两组的相关指标。结果联合组患儿的配合程度优于参考组（P＜0.05）。两组干预后的紧张和恐惧情绪评分均明显低于干预前，且联合组干预后的紧张和恐惧情绪评分均明显低于参考组（P＜0.05）。联合组的拔牙成功率明显高于参考组（P＜0.05）。联合组的家属总满意度明显高于参考组（P＜0.05）。联合组的不良事件发生率明显低于参考组（P＜0.05）。结论心理引导联合手摇滞留乳牙应用于基层医院6～10岁儿童无痛拔牙中具有多方面的积极作用，且并无任何风险，值得推广。

Abstract：Objective To investigate the application value of psychological guidance combined with hand shaking retained deciduous teeth in 6-10 years old children with retained deciduous teeth in primary hospital.Methods Sixty cases of 6-10 years old children given tooth extraction and treated in Department of Stomatology of the Fourth Hospital of Nanchang City from February 2016 to February 2018 were selected as the research objects and they were divided into the reference group and combination group according to the random number table generated by PEMS3.0 software package.The reference group was only given psychological guidance for infiltration anesthesia to extract a tooth,while the combination group was given psychological guidance combined with hand shaking retained deciduous teeth.The related indexes between the two groups were compared.Results The coordination degree in the combined group was better than that in the reference group (P<0.05).The score of tension and fear in the two groups were significantly lower than those before intervention,and the score of tension and fear in the combined group were significantly lower than those in the reference group (P<0.05).The success rate of tooth extraction in the combined group was significantly higher than that in the reference group (P<0.05).The total satisfaction degree of the family members in the combined group was significantly higher than that of the reference group (P<0.05).The incidence of adverse events in the combined group was significantly lower than that in the reference group (P<0.05).Conclusion Psychological guidance combined with hand shaking retained deciduous teeth applied in the painless extraction of 6-10 years old children in primary hospital as many positive effects,and it has no risk and is worth promoting.
