中医药对膀胱癌术后灌注化疗减毒增效的临床研究

摘要目的探讨中医药对膀胱癌术后灌注化疗减毒增效的临床效果。方法选择2013年3月～2015年3月我院收治的行经尿道膀胱肿瘤电切术（TURP）的早期浅表性膀胱癌患者60例，按随机数字表法将其分为试验组与对照组。对照组单纯予以化疗药物膀胱灌注，试验组加用辨证论治中药口服。比较两组治疗前后症状综合评分、不良反应发生情况及术后复发率。结果①两组患者治疗后症状综合评分均显著低于治疗前（P＜0.05），且试验组患者治疗后症状综合评分均显著低于对照组治疗后（P＜0.05）；②两组患者治疗后各种不良反应（发热、胃肠道反应、骨髓抑制、肝肾功能损害及皮疹脱发、轻度心电图改变）发生率比较，差异无统计学意义（P＞0.05），但试验组上述不良反应持续时间均显著短于对照组（P＜0.05）；③对照组患者术后1年内及3年内复发率分别为36.67% （11/30）、46.67%（14/30）；试验组患者术后1年内及3年内复发率分别为13.33%（4/30）、26.67%（8/30）。两组患者术后1年内及3年内复发率比较，差异有统计学意义（P＜0.05）。结论中医药对膀胱癌术后灌注化疗减毒增效的临床疗效显著，不良反应持续时间较短。

Abstract：Objective To explore the clinical effect of traditional Chinese medicine (TCM) on toxicity reducing and efficacy enhancing of infusion chemotherapy after bladder cancer surgery.Methods From March 2013 to March 2015，60 early-stage superficial bladder cancer patients undergone transurethral resection of the prostate (TURP) were admitted into our hospital and divided into experimental group and control group by the random number table.In the control group，only chemotherapy drug from bladder infusion was used，while in the experimental group，Chinese decoction based on syndrome differentiation and treatment was added.The comprehensive scoring of symptoms，occurrence of side effect，and postoperative recurrence rate before and after treatment between two groups were compared.Results The comprehensive scoring of symptoms after therapy in both groups was much less than that before treatment respectively(P＜0.05).The comprehensive scoring of symptoms after therapy in the experimental group was lower than that in the control group (P＜0.05).There were no statistical differences in side effects including fever，gastrointestinal side effect，bone marrow suppression，hepatic and renal functions damaging，rash，hair loss，or mild electrocardiogram change (P＞0.05).The duration of above mentioned side effects in the experimental group was greatly shorter in comparison with that in the control group (P＜0.05).In the control group，recurrence rate one or three years after surgery was 36.67%(11/30) and 46.67% (14/30) respectively，while in the experimental group，it was 13.33%(4/30) and 26.67%(8/30) respectively，which displayed statistical differences (P＜0.05).Conclusion TCM's effect of toxicity reducing and efficacy enhancing on infusion chemotherapy after bladder cancer surgery is remarkable in clinic with short duration of toxic and side effects.
