激光闭塞联合旋切术治疗下肢静脉曲张的临床疗效

摘要目的探讨激光闭塞联合旋切术治疗下肢静脉曲张的临床疗效。方法选取2013年1月～2015年6月我院收治的下肢静脉曲张患者80例作为研究对象。所有患者均经超声及临床症状检查确诊为下肢静脉曲张，根据患者手术类型将其分为对照组与观察组，其中对照组（35例）接受传统大隐静脉高位结扎手术，观察组（45例）采取激光闭塞联合旋切术治疗，比较两组患者治疗效果及手术用时、术中出血量、术后并发症等指标。结果观察组手术用时、术中出血量、住院时间、皮下瘀斑吸收时间明显短于对照组，差异有统计学意义（P＜0.05）；观察组术后并发症发生率明显低于对照组，且6个月随访观察组患肢静脉曲张复发率明显低于对照组，差异有统计学意义（P＜0.05）。结论激光闭塞联合旋切术治疗下肢静脉曲张疗效确切，并且手术医源性创伤较小，术后康复速度较快，切口数量少、术后遗留瘢痕较少、手术安全性相对较高，与传统大隐静脉结扎手术比较，具有明显优势，具有临床应用及推广价值。

Abstract：Objective To discuss the clinical curative effect of laser occlusion combined with atherectomy on the treatment of lower extremity varicose veins.Methods 80 cases with lower extremity varicose veins，which admitted in our hospital during January 2013 to June 2016，were selected as the research object.All patients were diagnosed for lower extremity varicose veins by ultrasound and clinical examination and divided into the observation group and the control group according to the type of surgery.35 cases in the control group were given the traditional great saphenous vein high ligation，and 45 cases in the observation group were given the laser occlusion combined with atherectomy treatment.Treatment effect and index such as operation time，intraoperative blood loss and postoperative complications of the two groups were compared.Results The operation time，intraoperative blood loss，hospital stay，subcutaneous ecchymosis absorption time of the observation group was significantly shorter than that of control group，the difference was significant statistically significant（P＜0.05）.Incidence of postoperative complications of observation group was obviously lower than that of control group，and 6 months follow-up limb varicose recurrence rate of the observation group was significantly lower than that of control group，the difference was significant statistically significant（P＜0.05）.Conclusion Laser occlusion combined with atherectomy on the treatment of lower extremity varicose veins has exact curative effect，less iatrogenic trauma，postoperative recovery fast，small incision，less postoperative scar left and the operation security is relatively high.Compared with the traditional great saphenous vein ligation has obvious advantages，which has clinical application and popularization value.
