惊恐障碍患者心理特征和心率变异性的表达及临床意义

摘要目的探讨惊恐障碍患者心理特征和心率变异性的表达及临床意义。方法选取广东省人民医院2013年6月～2014年12月接收的惊恐障碍患者60例为研究组，另选取同时期于广东省人民医院健康体检的60例健康人员为对照组，两组均采用心理问卷调查并借助心率变异性频谱对其心率变异性进行分析、对比。结果研究组患者均存在不同程度的焦虑，与对照组比较差异有统计学意义（P＜0.05）；对照组R-R间期标准差（SDNN）、间隔5 min间期标准差（SDANN）、相邻RR间期差异≥50 ms百分数（PNN50）、相邻R-R间期差值均方根（RMSSD）以及频域分析高频功率（HF）、低频功率（LF）、极低频功率（VLF），低高频功率比（LF/HF）等指标水平均高于研究组，差异有统计学意义（P＜0.01）。结论惊恐障碍患者心理状态不佳，影响其心率变异性，应根据患者的实际情况采取相关措施以改善预后。

Abstract：Objective To discuss the expression and clinical significance of psychological characteristics and heart rate variability in patients with panic disorder.Methods Sixty patients with panic disorder treated by Guangdong Provincial People′s Hospital from June 2013 to December 2014 were selected as the study group,and another sixty healthy people for physical examination in Guangdong Provincial People′s Hospital during the same period were selected as the control group.Both the two groups were given psychological questionnaire,and the heart rate variability was compared and analyzed by heart rate variability spectrum.Results The patients in the study group had different level of anxiety,and the differences were statistically significant compared with those in the control group(P<0.05).The indexes such as R-R interval standard deviation(SDNN),interval 5 min interval standard deviation(SDANN),adjacent to the diversity of R-R was greater than or equal to 50 ms percentage(PNN50),root mean square difference of the adjacent R-R interval (RMSSD)high frequency(HF),low frequency power(LF),extremely low frequency power(VLF),low frequency power contrast high frequency(LF/HF)in the control group were higher than those in the study group,with significant differences(P<0.01).Conclusion The mental state of panic disorder patients is not good,which affects the heart rate variability, and the relative measures should be taken according to the actual situationof the patients to improve the prognosis.
