经椎弓根椎体内植骨短节段内固定治疗胸腰椎压缩性骨折的效果分析

摘要目的观察经椎弓根椎体内植骨短节段内固定治疗胸腰椎压缩性骨折的临床效果。方法选取2013年1月～2016年6月我院收治入院的200例胸腰椎压缩性骨折患者为研究对象，根据骨折治疗方法的不同划分为两组，每组100例，对照组采用单纯短节段弓根钉棒系统内固定治疗，观察组在对照组治疗的基础上采用经椎弓根椎体内植骨治疗。对比分析两组手术情况、骨折恢复情况。结果两组患者手术时间、术中出血量、住院时间、骨折愈合时间比较，差异无统计学意义（P＞0.05）。观察组患者内固定断裂、松动等发生率明显低于对照组，差异有统计学意义（P＜0.05）。观察组术后VAS疼痛评分为（1.7±0.5）分，对照组为（4.9±2.4）分，差异有统计学意义（P＜0.05）。观察组术后椎体丢失高度为（4.1±1.3）%，对照组为（7.9±1.8）%，差异有统计学意义（P＜0.05）。结论经椎弓根椎体内植骨短节段内固定治疗胸腰椎压缩性骨折效果显著，可减轻患者的疼痛，术后患者骨折功能恢复良好。

Abstract：ObjectiveTo observe the clinical effects of transpedicular intervertebral bone graft and short segment internal fixation in the treatment of thoracolumbar vertebral compression fractures.MethodsAltogether 200 cases of patients with thoracolumbar vertebral compression fractures who were admitted into the hospital from January 2013 to June 2016 were selected as research object and divided into control group and observation group according to the different treatmentmethods,with 100 cases in each group,patients in the control group was treated simply with the short segment pedicle screw rod fixation system,while patients in the observation group was treated with transpedicular intervertebral bone graft on the basis of the control group.Surgical outcomes and fracture recovery of two groups was compared.ResultsNo obvious statistical difference was seen between operative time,intraoperative blood loss,length of stay and fracture recovery time of two groups(P＞0.05).The incidence rate rapture and loosening of internal fixation in the observation group showed great lower than control group,with a statistical difference (P＜0.05).The postoperative visual analogue score (VAS)pain score in the observation group and control group was (1.7±0.5)scores,(4.9±2.4) scores,with a statistical difference between two groups(P＜0.05).The vertebral body lost height postoperatively in the observation group was(4.1±1.3)%,with (7.9±1.8)%in the control group,and the difference was statistically significant (P＜0.05).ConclusionTranspedicular intervertebral bone graft and short segment internal fixation demonstrates in the treatment of thoracolumbar vertebral compression fractures has excellent effect,which significantly reduces patients′pain and promotes function recovery of postoperative fracture.
