神经阻滞复合不同的静脉全麻药物组合用于婴幼儿腹股沟疝疝囊高扎麻醉效果分析

摘要目的探讨神经阻滞复合不同的静脉全麻药物组合用于婴幼儿腹股沟疝疝囊高扎的麻醉效果。方法选择2014年10月～2016年8月来150例来我院行腹股沟疝疝囊高扎的6个月～3周岁的患儿，将其按随机数字表法分为3组：Ⅰ组芬太尼2 μg/kg和异丙酚2.5 mg/kg静脉注射；Ⅱ组氯胺酮2 mg/kg和异丙酚2.5 mg/kg静脉注射；Ⅲ组氯胺酮2 mg/kg静脉注射；均待患儿意识消失后，用0.5%利多卡因和0.25%罗哌卡因混合液0.3 ml/kg，行患侧髂腹下及髂腹股沟神经阻滞，患儿若在手术期间出现体动反应，即Ⅰ、Ⅱ组予以异丙酚0.5～1.0 mg/kg，Ⅲ组予以氯胺酮1 mg/kg静脉注射至体动反应消失。比较不同时间点的心率（HR）、呼吸（RR）及经皮血氧饱和度（SpO2）变化情况及苏醒时间、并发症。结果Ⅰ、Ⅲ组各时间点的HR同术前（T0）比较差异有统计学意义（P＜0.05）；各组同时间点（T1、T2、T3）的HR比较，差异有统计学意义（P＜0.05）；三组的RR、SpO2比较差异无统计学意义（P＞0.05）。Ⅲ组的苏醒时间最长，Ⅰ组最短且镇静评分最满意；并发症和不良反应Ⅲ组最多，Ⅰ组最少（P＜O.05）。结论芬太尼2 μg/kg和异丙酚2.5 mg/kg静脉注射复合神经阻滞的麻醉效果最佳。

Abstract：Objective To study the anesthetic effect of nerve block combined with different intravenous general anesthetics on infants′inguinal hernia sac high ligation.Methods 150 children from 6 months to 3 years of age given high ligation of inguinal hernia sac in our hospital from October 2014 to August 2016 were selected and they were randomly divided into 3 groups according to the random number table method,groupⅠ:intravenous injection of Fentanyl 2 μg/kg and Propofol 2.5 mg/kg;groupⅡ:intravenous injection of Ketamine 2 mg/kg and Propofol 2.5 mg/kg; groupⅢ:intravenous injection of Ketamine 2 mg/kg.When children′s awareness disappeared,0.3 ml/kg mixture of 0.5% Lidocaine and 0.25%Ropivacaine was used to perform ipsilateral iliohypogastric and ilioinguinal nerve block.If the body reaction occurred during the operation,then groupⅠ and groupⅡwere given intravenous injection of Propofol 0.5-1.0 mg/kg,and groupⅢwas given Ketamine 1 mg/kg until the body reaction disappeared;the change of heart rate (HR),respiratory rate (RR)and percutaneous oxygen saturation (SpO2)at different time points,and awakening time and complications were observed and compared.Results The change of HR in groupⅠ and groupⅢ at different time points were different with those before operation (T0),and the difference was statistically significant(P＜0.05).HR was compared at the same time points in each group:the differences at T1,T2,T3were statistically significant(P＜0.05);the change of RR and SpO2among the three groups had no statistical difference (P＞0.05);the awakening time in groupⅢwas the longest.The awakening time was the shortest in groupⅠ,and the sedation score was the most satisfactory; complications and adverse reactions were the most in groupⅢ and the least in groupⅠ.Conclusion Intravenous injection combined with nerve block anesthesia by Fentanyl 2 μg/kg and Propofol 2.5 mg/kg is the best.
