一例以窒息为表现的大量先天性乳糜胸新生儿诊治过程及文献复习

摘要为探索新生儿大量先天性乳糜胸的急救、早期诊断及治疗方法，本文分析一例以出生时重度窒息为主要临床表现的大量先天性乳糜胸新生儿的诊治过程，并通过文献复习，总结先天性乳糜胸的诊治经验和研究进展。结果显示，本例患儿于产前确诊有大量胸腔积液，经早期复苏、呼吸支持、胸腔减压、静脉营养等综合治疗，该患儿痊愈出院，通过分析相关文献，对新生儿先天性乳糜胸的病因、诊断及治疗方法进行总结。可见先天性乳糜胸是新生儿呼吸窘迫综合征鉴别诊断中的疾病，死亡率高，产前诊断、分娩时的积极复苏，出生后的综合治疗可有效降低死亡率。

Abstract：To explore resuscitation,early diagnosis and treatment of newborn with large number of congenital chylothorax,this paper has analyzed a case of severe asphyxia at birth as the main clinical manifestations of congenital chylothorax neonatal diagnosis and treatment process，and through literature review,summarizes the experience of diagnosis and treatment of congenital chylothorax.ResultsShows that the cases in the prenatal diagnosis of a large number of pleural effusion,the prompt resuscitation,respiratory support,chest decompression,parenteral nutrition,the children recovered and discharged from the hospital,and analyze the related literature,summarize the causes of neonatal congenital chylothorax,diagnosis and treatment.So massive congenital chylothorax is easy to cause severe neonatal asphyxia,resuscitation,ventilator,chest drainage and nutritional support are the main treatments.
