Abstract:ObjectiveTo discuss the causes, treatments and prognosis of anastomotic leakage after one-stage repair of esophageal atresia(EA)and/or tracheoesophageal fistula(TEF).Methods168 EA and/or TEF cases with one-stage repair in this medical center from January 2007 to January 2017 were retrospectively studied.Relevant factors such as the incidence of leakage, the potential cause and the prognosis of these cases were investigated.ResultsIn 168 cases,36 cases(21.43%)suffered leakage postoperation.Among them,8 leakage(22.22%)occurred in 4 days postoperation,26 leakage(72.22%)occurred between 5 days and 10 days, and 2 leakage(5.56%)occurred after 10 days(P<0.05).Underwent treatment,19 cases(52.78%)recovered in 1-2 weeks,11 cases(30.56%)recovered in 2-4 weeks, and the other 4 cases(11.11%)recovered in 4-6 weeks.Among the 43 cases of typeⅢa,22 cases(51.16%)suffered leakage, however, among the 117 caese ofⅢb,only 14 cases(11.97%)suffered leakage, which had significant difference(P<0.05).Patients with different birth weight, congenital cardiac abnormalitis, gastroesophageal reflux and anastomotic stricture had different incidence of anastomotic leaking, but there was no statistically significant difference(P>0.05).ConclusionAnastomotic leakage usually occurs between 5-10 days postoperation. The Gross classification is the influence factor of the incidence of anastomotic leakage after one-stage repair of EA and/or TEF.Most of the anastomotic leakage recovered in 4 weeks postoperation after conservative treatments.
