Abstract:Objective To study the characteristics of preoperative ultrasonography in patients with cervical lymph node metastasis (CLNM) of papillary thyroid carcinoma (PTMC). Methods From January 2016 to December 2019, the clinical data of 100 patients with PTMC confirmed by postoperative pathology in our hospital were analyzed retrospectively,31 patients developed CLNM among them. All patients underwent ultrasound examination before operation. The preoperative ultrasound examination data were analyzed.The difference between CLNM and non-CLNM preoperative ultrasound image characteristics was analyzed by univariate analysis, and the independent risk factors of CLNM were analyzed by multivariate Logistic regression. Results Univariate analysis showed that there were no significant difference between the two groups in the location, aspect ratio, shape, internal echo and internal echo components (P>0.05). There were significant differences between the two groups in maximum diameter of cancer focus, definition of nodal boundary, microcalcification, internal blood flow signal and extracapsular invasion of thyroid (P<0.05). The results of multivariate Logistic regression analysis showed that the maximum diameter of the cancerous foci >5 mm, microcalcification, and extrathyroidal invasion were independent preoperative ultrasound risk signs of CLNM (P<0.05). Conclusion Preoperative ultrasound signs have a certain predictive value for CLNM in patients with PTMC, among which the maximum diameter of the cancerous foci >5 mm, microcalcification and extrathecal thyroid invasion are the high-risk features of CLNM.
