食管支架置入联合适形放疗治疗局部晚期食管癌的临床效果

摘要目的探讨食管支架置入联合适形放疗治疗局部晚期食管癌的临床效果。方法收集我院肿瘤科2014年1月～2016年4月共60例局部晚期食管癌患者入组，分为试验组及对照组，每组30例。试验组采用支架置入后再适形放疗，对照组采用单纯适形放疗，两组患者放疗方法及剂量相同，治疗结束后比较两组患者的近期疗效（CR+PR），6个月、1年生存率及治疗相关并发症发生率。结果试验组的近期总有效率为93.3%，高于对照组的70.0%，两组比较差异有统计学意义（P＜0.05）；试验组的6个月、1年生存率为90.0%、76.7%，高于对照组的66.6%、50.0%，两组比较差异有统计学意义（P＜0.05）。试验组的Ⅱ级以上胸痛发生率为26.7%，对照组为6.6%，试验组与对照组的食管再狭窄发生率分别为3.3%、23.3%，食管穿孔发生率分别为0、13.3%，两组比较差异有统计学意义（P＜0.05），两组的食管炎发生率差异无统计学意义（P＞0.05）。结论与单纯放疗比较，食管支架置入联合适形放疗的近期总有效率及1年生存率更高，食管穿孔及食管再狭窄发生率更低，可明显改善晚期食管癌患者的营养状况，提高其生活质量。

Abstract：ObjectiveTo investigate the clinical effect of esophageal stent placement combined with conformal radiotherapy in the treatment of local advanced esophageal cancer.MethodsA total of 60 patients with local advanced esophageal cancer were enrolled in the oncology department from January 2014 to April 2016.They were divided into experimental group and control group,and there were 30 cases in each group.Experimental group was treated with stent placement,and then given the conformal radiotherapy,while control group was treated with conformal radiotherapy alone.The radiation methods and dosage in the two groups were the same.Short-term effect(CR+PR),6 months and 1 year survival rate and incidence of treatment-related complications were compared in patients between the two groups after treatment.ResultsThe total effective rate of short-term in experimental group was 93.3%,which was higher than that of control group(70.0%),and the difference was statistically significant(P＜0.05).6 months and 1 year survival rate of experimental group was 90.0%and 76.7%respectively,which were higher than those of control group(66.6%and 50.0%),and the differences were statistically significant(P＜0.05).The incidence of chest pain above classⅡ in experimental group and control group was 26.7%and 6.6%respectively,and the incidence of esophageal restenosis in experimental group and control group was 3.3%and 23.3%respectively after treatment,the incidence of esophageal perforation were 0 and 13.3%respectively,and there was a statistical difference between the two groups(P＜0.05).There was no statistical difference of incidence of esophagitis between the two groups (P＞0.05).ConclusionCompared with simple radiotherapy,esophageal stent placement combined with conformal radiotherapy has a higher the total effective rate of short-term and 1 year survival rate,and a lower the incidence of esophageal perforation and esophageal restenosis and can significantly improve the nutritional status and life quality of patients with advanced esophageal cancer.
