Comparison of the application effect of modified keyhole approach and conventional bone flap approach in microvascular decompression of trigeminal neuralgia

Abstract:Objective To compare the application effect of modified keyhole approach and conventional bone flap approach in microvascular decompression(MVD)of trigeminal neuralgia.Methods The medical records of 57 patients with trigeminal neuralgia who treated with MVD in the Department of Neurosurgery of Central Hospital of Huanggang City from February 2017 to June 2019 were retrospectively analyzed.They were divided into the conventional bone flap approach group(30 cases)and the modified keyhole approach group(27 cases)according to different surgical methods.The conventional bone flap approach group was treated with MVD through the conventional bone flap approach,and the modified keyhole approach group was treated with MVD through the modified keyhole approach.Clinical efficacy between the two groups was compared.Follow-up was performed for 12 months after surgery,and the incidence of complications such as intracranial hematoma,cranial nerve palsy,subcutaneous effusion,intracranial infection,cerebrospinal fluid leakage,dizziness/hearing loss of the two groups were recorded in detail.Results The total effective rate of the modified keyhole approach group(88.89%)was higher than that of the conventional bone flap approach group(66.67%),and there was a statistical significance(P<0.05).The total incidence of complications such as intracranial hematoma,cranial nerve palsy,subcutaneous effusion,intracranial infection,cerebrospinal fluid leakage,vertigo/hearing loss of the modified keyhole approach group(14.81%)was lower than that of the conventional bone flap approach group(36.67%),and there was a statistical significance(P<0.05).Conclusion The therapeutic effect of modified keyhole approach in MVD of trigeminal neuralgia is superior to the conventional bone flap approach,and the incidence of postoperative complications of modified keyhole approach is significantly lower than that of conventional bone flap approach.
