七氟醚与丙泊酚在麻醉/脑电意识深度监测系统监测下腹腔镜手术深麻醉苏醒期的效果比较

摘要目的探讨七氟醚与丙泊酚在麻醉/脑电意识深度监测系统（Narcotrend）监测下腹腔镜手术深麻醉苏醒期的效果。方法回顾性分析2020年1～8月于广东省东莞市寮步医院接受Narcotrend监测下普外或妇科腹腔镜手术的60例患者的临床资料，按照术中深麻醉所用药物的不同将其分为七氟醚组（28例）与丙泊酚组（32例）。七氟醚组患者使用七氟醚和瑞芬太尼维持麻醉，丙泊酚组患者使用丙泊酚和瑞芬太尼维持麻醉。比较两组患者的血流动力学指标、功能恢复时间、视觉模拟量表（VAS）评分、最大Ricker镇静躁动评分及24 h恢复质量（QoR-40）评分。结果两组患者术前及苏醒期的心率（HR）、平均动脉压（MAP）比较，差异无统计学意义（P＞0.05）。丙泊酚组患者的恢复自主呼吸时间、呼之睁眼时间、拔管时间、恢复定向力时间短于七氟醚组，差异有统计学意义（P＜0.05）。丙泊酚组患者苏醒期的VAS评分、最大Ricker镇静躁动评分低于七氟醚组，QoR-40评分高于七氟醚组，差异有统计学意义（P＜0.05）。结论 Narcotrend监测下丙泊酚与七氟醚在腹腔镜手术深麻醉中效果都较稳定，采用丙泊酚深麻醉的恢复时间较短，恢复质量较高，患者的躁动较少，疼痛水平较低，更有利于患者苏醒后的功能恢复。

Abstract：Objective To investigate the effect of Sevoflurane and Propofol during the recovery period of deep anesthesia in laparoscopic surgery under Narcotrend monitoring.Methods The clinical data of 60 patients receiving general or gynecological laparoscopic surgery under Narcotrend monitoring in Liaobu Hospital of Dongguan City,Guangdong Province from January to August,2020 were retrospectively analyzed.The patients were divided into Sevoflurane group(28 cases)and Propofol group(32 cases)according to the different drugs used in deep anesthesia.Sevoflurane group was given Sevoflurane and Remifentanil to maintain anesthesia,while Propofol group was given Propofol and Remifentanil to maintain anesthesia.Hemodynamic indexes,functional recovery time,visual analogue scale(VAS)score,maximum Ricker calming agitation score and 24 h recovery quality(QOR-40)score were compared between the two groups.Results There were no statistically significant differences in the heart rate(HR)and mean arterial pressure(MAP)between the two groups before operation and during the recovery period(P＞0.05).The recovery time of spontaneous respiration,breathing and eye-opening time,extubation time,and recovery time of orientation in the Propofol group were shorter than those in the Sevoflurane group,and the differences were statistically significant(P＜0.05).VAS score and maximum Ricker sedative agitation score in the Propofol group were lower than those in the Sevoflurane group,and QOR-40 score was higher than that in the Sevoflurane group,and the differences were statistically significant(P＜0.05).Conclusion Under Narcotrend monitoring,the effect of Propofol and Sevoflurane in laparoscopic deep anesthesia is relatively stable.The recovery time in the recovery period of deep anesthesia with Propofol is shorter,the recovery quality is higher,the patients′restlessness is less,the pain level is lower,and it is more conducive to the functional recovery of patients after awakening.
