基层医院糖尿病患者血糖稳定性、治疗费用及其对生活质量影响的调查分析

摘要目的探讨基层医院糖尿病患者血糖稳定性、治疗费用及其对生活质量的影响。方法选取2017年3月～2019年7月社区2 型糖尿病（T2DW）患者206 例，收集、统计患者一般资料，评价患者血糖稳定性和生活质量，分析影响血糖稳定性与生活质量的相关因素。结果 206 例T2DW 患者中，血糖稳定133 例（64.56%），血糖不稳定73 例（35.44%）；高血压、高脂血症、吸烟史、饮酒史、饮食控制差、超重或肥胖、从不锻炼是影响血糖稳定性的独立危险因素（OR≥1，P＜0.05）；206 例T2DW 患者中，生活质量高146 例（70.87%），生活质量不高60 例（29.13%）；治疗费用＞5000 元91 例（44.17%），≤5000 元115 例（55.83%）；年龄≥60 岁、血糖不稳定、治疗费用＞5000 元、饮食控制差、超重或肥胖是影响生活质量的独立危险因素（OR≥1，P＜0.05）。结论基层医院糖尿病患者治疗费用较高、血糖稳定性较好、生活质量较高，其中高血压、高脂血症、吸烟史、饮酒史、饮食控制差、超重或肥胖、从不锻炼是影响血糖稳定性的独立危险因素，而血糖不稳定、治疗费用＞5000 元、饮食控制差、超重或肥胖是影响生活质量的独立危险因素，临床需加以重视。

Abstract：Objective To explore the stability of blood glucose, the cost of treatment and the impact on the quality of life of diabetes patients in primary hospitals. Methods A total of 206 patients with type 2 diabetes mellitus (T2DW) in the community from March 2017 to July 2019 were selected. The general data of patients were collected and counted,the stability of blood glucose and quality of life were evaluated, and the relevant factors affecting the stability of blood glucose and quality of life were analyzed. Results Among the 206 T2DW patients, 133 (64.56%) had stable blood glucose and 73 (35.44%) had unstable blood glucose; hypertension, hyperlipidemia, smoking, drinking, poor diet control,overweight or obesity, and never exercising were independent risk factors affecting blood sugar stability (OR≥1, P＜0.05). Among 206 T2DW patients, 146 (70.87%) had high quality of life and 60 (29.13%) had low quality of life. A total of 91 cases (44.17%) had treatment cost ＞5000 yuan and 115 cases (55.83%) had treatment cost ≤5000 yuan.Age ≥60 years old, unstable blood sugar, treatment cost ＞5000 yuan, poor diet control, overweight or obesity are independent risk factors affecting quality of life (OR≥1, P＜0.05). Conclusion Diabetes patients in primary hospitals have higher treatment costs, better blood sugar stability and higher quality of life. Hypertension, hyperlipidemia, smoking history, drinking history, poor diet control, overweight or obesity, and never exercising are independent risk factors affecting blood sugar stability, while unstable blood sugar, treatment cost ＞5000 yuan, poor diet control, overweight or obesity are independent risk factors affecting quality of life, which should be paid attention to clinically.
