无缝线中切口白内障囊外摘除术治疗硬核白内障的临床效果

摘要目的探讨无缝线中切口白内障囊外摘除术（SLIMCE）治疗硬核白内障的临床效果。方法选取2016年4月～2018年4月江西省分宜县人民医院收治的200例硬核白内障患者作为研究对象，按照随机数字表法分为对照组（100例）和观察组（100例）。对照组采用传统大切口囊外摘除术治疗，观察组采用无缝线中切口白内障囊外摘除术治疗。比较两组患者术后1周、1个月的裸眼视力情况及治疗前后的角膜散光度、角膜内皮细胞损伤情况。结果术后1周，观察组患者裸眼视力在0.1～0.4水平的占比低于对照组，裸眼视力在＞0.4～0.9水平的占比高于对照组，裸眼视力在＞0.9水平的占比高于对照组，差异均有统计学意义（P＜0.05）；术后1个月，观察组患者裸眼视力在＞0.4～0.9水平的占比低于对照组，裸眼视力在＞0.9水平的占比高于对照组，差异均有统计学意义（P＜0.05）；观察组患者术后1周的散光度低于术前，术后1个月的散光度低于术前及对照组，术后1周、1个月的内皮细胞减少量低于术前及对照组，差异均有统计学意义（P＜0.05）。结论硬核白内障患者采用SLIMCE治疗效果显著，能够更好地改善患者的视力状况，提升治疗的安全性。

Abstract：Objective To explore the clinical effect of suturless large incision manual cataract extraction (SLIMCE)in the treatment of hard nucleus cataract.Methods A total of 200 cases of hard nucleus cataract patients admitted to Fenyi People′s Hospital of Jiangxi Province from April 2016 to April 2018 were selected as the research objects,they were divided into control group (100 cases)and observation group (100 cases)according to random number table method.The control group was treated with traditional large incision extracapsular cataract extraction,while the observation group was treated with SLIMCE.The visual acuity,corneal astigmatism and corneal endothelial cell injury were compared between the two groups one week and one month after operation.Results One week after operation,the percentages of naked eye vision in the observation group were lower than those in the control group at 0.1-0.4 level,higher than those in the control group at＞0.4-0.9 level,and higher than those in the control group at＞0.9 leve,the differences were statistically significant(P＜0.05).One month after operation,the proportion of naked eye vision in the observation group was lower than that in the control group at the level of＞0.4-0.9,and the proportion of naked eye vision at the level of＞0.9 was higher than that in the control group,the differences were statistically significant(P＜0.05).The astigmatism of the observation group was lower than that of the preperative one week after operation,the astigmatisn one month after operation was lower than that preoprathve and dntol group,the endothelial cell reduction of the observation group was lower than that inpreaperative and control group one week and one month after operation,the differences were statistically significant(P＜0.05).Conclusion SLIMCE is an effective method for hard nucleus cataract patients,which can improve their visual acuity and enhance the safety of treatment.
